บทคัดย่อ

บทนำ: ผู้ป่วยโรคเรื้อรังที่ต้องคาสายสวนปัสสาวะระยะยาวมีความเสี่ยงที่จะเกิดการติดเชื้อทางเดินปัสสาวะ และภาวะแทรกซ้อนจากการติดเชื้อในกระแสเลือด ส่งผลต่ออัตราการเสียชีวิตและค่าใช้จ่ายด้านสุขภาพเพิ่มขึ้น ดังนั้นบทบาทของพยาบาลในการดูแลต่อเนื่องจากโรงพยาบาลสู่บ้านจึงมีความสำคัญ   

วัตถุประสงค์: เพื่อสังเคราะห์และนำเสนอแนวทางการพยาบาลผู้ป่วยโรคเรื้อรังที่ได้รับการคาสายสวนปัสสาวะที่บ้าน โดยบูรณาการแนวคิด D-METHOD และการพยาบาลทางไกล เพื่อป้องกันการติดเชื้อทางเดินปัสสาวะจากการคาสายสวนปัสสาวะในผู้ป่วยโรคเรื้อรัง

ประเด็นสำคัญ: การนำหลัก D-METHOD มาใช้ โดยเน้นการป้องกันการติดเชื้อทางเดินปัสสาวะที่บ้าน ร่วมกับการพยาบาลทางไกลมาประยุกต์ใช้ โดยติดตามอาการผ่านโทรศัพท์มือถือหรือไลน์แอปพลิเคชัน โดยมีความถี่ในการติดตามหลังจากจำหน่ายออกจากโรงพยาบาลอย่างน้อย 1 เดือน เพื่อประเมินภาวะสุขภาพ การให้คำปรึกษา แก้ปัญหา การติดตามอาการ การประเมินปัจจัยที่จะทำให้เกิดการติดเชื้อทางเดินปัสสาวะ และวางแผนการดูแลให้เหมาะสมตามบทบาทของพยาบาลผู้ป่วยนอก พยาบาลเยี่ยมบ้าน และพยาบาลชุมชน                        

สรุป: การบูรณาการ D-METHOD และการพยาบาลทางไกลตามบทบาทของพยาบาลผู้ป่วยนอก พยาบาลเยี่ยมบ้าน และพยาบาลชุมชน ส่งเสริมให้ผู้ป่วยมีความรู้ ความมั่นใจในการดูแลตนเองและสนับสนุนการดูแลต่อเนื่องของผู้ป่วยโรคเรื้อรังที่คาสายสวนปัสสาวะที่บ้าน 

ข้อเสนอแนะ: การบูรณาการ D-METHOD และการพยาบาลทางไกล สอดคล้องกับบริบทของสังคมที่เปลี่ยนแปลงไปสามารถนำไปประยุกต์ใช้งานได้จริง ตอบสนองต่อความต้องการของผู้ให้บริการและผู้รับบริการ
