Nursing Care for Patients with Chronic Diseases Who Have Received Urinary Catheterization at Home According to The D-METHOD Concept and Tele-Nursing

Introduction: Patients with chronic diseases who require long-term catheterization are at risks of urinary tract infections and bloodstream infections, leading to increased mortality rates and healthcare costs. Therefore, nurses play an important role in continuity of care from hospital to home.
Objectives: To synthesize and present a nursing care framework integrating the D-METHOD concept and tele-nursing for the prevention of catheter-associated urinary tract infections among patients with chronic conditions.
Key Issues: This framework implements the D-METHOD approach alongside tele-nursing modalities, utilizing mobile communication or the LINE application to conduct patient follow-ups for at least one-month post-discharge. This process includes health status assessment, counseling, problem-solving, symptom monitoring, evaluation of risk factors for urinary tract infections, and appropriate care planning, in accordance with the roles of outpatient, home care, and community nurses.
Conclusions: Integrating the D-METHOD framework with tele-nursing across outpatient, home care, and community nursing contexts may enhance patient knowledge and self-care confidence. It supports the ongoing care of patients with chronic diseases who have urinary catheters at home.
Implications: The integration of the D-METHOD framework and tele-nursing aligns with contemporary healthcare contexts. This adaptable responds directly to the needs of both healthcare providers and care recipients.
