บทคัดย่อ
วัตถุประสงค์ : 1) ศึกษาแบบแผนการสั่งยาดอมเพอริโดนเพื่อกระตุ้นน้ำนมในหญิงให้นมบุตรในโรงพยาบาลตามคำแนะนำการใช้ยาในคู่มือโครงการโรงพยาบาลส่งเสริมการใช้ยาอย่างสมเหตุผล และ 2) ศึกษาลักษณะของหญิงให้นมบุตรและทารกที่มารดาได้รับยาดอมเพอริโดนเพื่อกระตุ้นน้ำนม วิธีการ: งานวิจัยนี้เป็นการวิจัยเชิงพรรณนาแบบภาคตัดขวางในหญิงให้นมบุตรที่คลอดบุตรจำนวน 394 รายในช่วง 1 ตุลาคม พ.ศ.2556 ถึง 30 กันยายน พ.ศ.2558 การศึกษาเก็บข้อมูลจากแฟ้มประวัติผู้ป่วยในและฐานข้อมูลอิเล็กทรอนิกส์ของโรงพยาบาลโดยใช้แบบเก็บข้อมูลที่ผู้วิจัยสร้างขึ้นเพื่อเก็บข้อมูลลักษณะก่อนการคลอด ลักษณะการคลอด ข้อมูลสุขภาพทารก ข้อมูลการสั่งยาดอมเพอริโดนและยาอื่น และข้อมูลระดับการไหลของน้ำนม ผลการวิจัย: ความชุกของการได้รับยาดอมเพอริโดนเพื่อกระตุ้นน้ำนมในหญิงให้นมบุตร คือ ร้อยละ 8.9 แบบแผนการสั่งยาดอมเพอริโดนครั้งแรก คือ ครั้งละ 20 มิลลิกรัม วันละ 4 ครั้ง (วันละ 80 มิลลิกรัม) (ร้อยละ 82.9) ความชุกของการได้รับยาดอมเพอริโดนภายใน 10 วันหลังคลอด คือ ร้อยละ 8.9 ส่วนใหญ่ได้รับยาวันที่ 2 หลังคลอด (ร้อยละ 57.2) ได้รับยาต่อเนื่อง 1 วัน และ 2 วัน (ร้อยละ 42.9 และ ร้อยละ 20.0 ตามลำดับ) หญิงให้นมบุตรร้อยละ 80 ได้รับยาดอมเพอริโดนเมื่อน้ำนมไหลแล้ว หญิงให้นมบุตร 1 ราย ได้รับยาดอมเพอริโดนร่วมกับยาเลโวไทรอกซิน สรุป: มีการสั่งยาดอมเพอริโดนเพื่อกระตุ้นน้ำนมในหญิงให้นมบุตรในโรงพยาบาลในอัตราที่สูง และสั่งยาขนาดเริ่มต้นสูงมากกว่า 30 มก.ต่อวัน การสั่งยาดอมเพอริโดนส่วนใหญ่เป็นการได้รับยาโดยไม่มีข้อบ่งใช้ จึงควรมีการจัดการความเสี่ยงหลายด้านร่วมกัน และส่งเสริมให้แพทย์สั่งใช้ยาอย่างสมเหตุผล
