คุณภาพชีวิตด้านสุขภาพของผู้ป่วยโรคเรื้อรังในโรงพยาบาลชุมชน: กรณีศึกษาโรงพยาบาลบ้านไผ่ จังหวัดขอนแก่น

บทคัดย่อ

วัตถุประสงค์: เพื่อศึกษาคุณภาพชีวิตด้านสุขภาพและปัจจัยต่างๆ ที่สัมพันธ์กับคุณภาพชีวิตด้านสุขภาพของผู้ป่วยโรคเรื้อรังในโรงพยาบาลชุมชนแห่งหนึ่ง วิธีการ: การวิจัยนี้เป็นการศึกษาเชิงสำรวจภาคตัดขวาง ตัวอย่างเป็นผู้ป่วย 420 คน ที่มารับการรักษาที่คลินิกโรคเรื้อรัง โรงพยาบาลบ้านไผ่ จังหวัดขอนแก่น ในช่วงเดือนมีนาคมถึงเดือนมิถุนายน พ.ศ. 2561 การเลือกตัวอย่างใช้วิธีการเลือกอย่างเป็นระบบแบบแบ่งชั้นภูมิอย่างไม่มีสัดส่วน เครื่องมือวิจัยเป็นแบบสอบถามประเมินสถานะสุขภาพทั่วไป (9-THAI) โดยให้ผู้ป่วยตอบแบบสอบถามด้วยตนเอง ผลการวิจัย: ผู้ป่วยโรคเรื้อรังมีคุณภาพชีวิตอยู่ในระดับปานกลางทั้งมิติด้านกายและใจ (Physical Health Score: PHS=34.09, Mental Health Scores: MHS=36.54, คะแนนทั้งสองเป็น T scores ที่มีค่าเฉลี่ย 50±10)  ผู้ป่วยโรคความดันโลหิตสูงมีคะแนนเฉลี่ย PHS และ MHS สูงสุดเท่ากับ 39.72 และ 39.54 ตามลำดับ ส่วนผู้ป่วยโรคหัวใจมีคะแนนเฉลี่ย PHS ต่ำสุดเท่ากับ 28.73 ผู้ป่วยเบาหวานชนิดที่ 2 และผู้ป่วยเบาหวานชนิดที่ 2 ร่วมกับความดันโลหิตสูงมีคะแนนเฉลี่ย MHS ต่ำสุดเท่ากับ 32.95 การประเมินในภาพรวมพบว่า ผู้ป่วยส่วนใหญ่มีสุขภาพเหมือนกันกับปีที่แล้ว (ร้อยละ 30.5) แต่สุขภาพแย่กว่าผู้ที่ไม่มีโรคประจำตัวเล็กน้อย (ร้อยละ 28.6) การวิเคราะห์ถดถอยพหุคูณพบว่า ปัจจัยที่สัมพันธ์กับ PHS ได้แก่ โรคความดันโลหิตสูง โรคไทรอยด์ โรคหืด อายุ และสถานภาพสมรส ปัจจัยที่สัมพันธ์กับ MHS ได้แก่ อายุ สถานภาพสมรส และความเพียงพอของรายได้ต่อเดือน สรุป: ผลการศึกษาครั้งนี้สามารถใช้เป็นข้อมูลในการวางแผนการรักษาและจัดลำดับความสำคัญในการจัดการผู้ป่วยโรคเรื้อรังแต่ละโรค โดยคำนึงถึงคุณภาพชีวิตด้านสุขภาพของผู้ป่วยได้อย่างเหมาะสม
