บทคัดย่อ

การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาลักษณะทางกายวิภาคและจำแนกรูปแบบการแตกแขนงของหลอดเลือดแดง internal iliac artery (IIA) ในร่างดองคนไทย จำนวน 22 ร่าง (44 ข้าง) โดยอาศัยเกณฑ์การจำแนกตาม Adachi classification ผลการศึกษาพบว่า จุดกำเนิดของหลอดเลือดแดง IIA ส่วนใหญ่ตรงกับหมอนรองกระดูกสันหลังระดับ L5–S1 (45.5%) และจุดที่หลอดเลือดแยกออกเป็นแขนง anterior division และ posterior division ส่วนใหญ่ตรงกับกระดูกสันหลังระดับ S1 (43.2%) ความยาวเฉลี่ยของหลอดเลือดแดง IIA เท่ากับ 37.96 ± 12.78 มม. (12.20–71.57 มม.) ไม่มีความแตกต่างอย่างมีนัยสำคัญทางสถิติระหว่างซีกขวา–ซ้ายและระหว่างเพศชาย–หญิง รูปแบบการแตกแขนงที่พบบ่อยที่สุดคือ Type I (77.27%) รองลงมา Type II (15.91%) และ Type III (6.82%) โดยไม่พบ Type IV และ Type V ผลการศึกษานี้สอดคล้องกับรายงานในประชากรทั่วโลกที่พบว่า Type I เป็นรูปแบบที่พบมากที่สุด บ่งชี้ว่าลักษณะทางกายวิภาคของหลอดเลือดแดง IIA ในคนไทยสอดคล้องกับภาพรวมของประชากรโลก แต่ช่วงความยาวที่กว้างมากสะท้อนความแปรผันรายบุคคลสูง ผลการศึกษานี้จึงเป็นฐานข้อมูลทางกายวิภาคอ้างอิงสำคัญสำหรับแพทย์ผู้ทำการผ่าตัดและรังสีแพทย์ในบริเวณอุ้งเชิงกราน ช่วยลดความเสี่ยงต่อการบาดเจ็บต่อหลอดเลือดและเพิ่มความปลอดภัยในการผ่าตัดบริเวณอุ้งเชิงกราน
