A A Systematic Review and Meta-Analysis of the Effectiveness of Powered Versus Manual Toothbrushing for Oral Health in Children

Abstract

Objectives: To systematically review and conduct meta-analysis to determine the effectiveness of powered versus manual toothbrush for oral health in children. Method: The study searched published articles from electronic databases including Pubmed, The Cochrane Library, Scopus, ThaiLis, TCI, HITAP, Thai thesis databases of various universities as well as hand searching from reference lists in the previous systematic reviews. The researchers systematically searched studies published from inception to October 2018 and selected randomized controlled trials comparing the effectiveness of powered versus manual toothbrush for oral health in children. The quality of included studies was assessed according to Cochrane risk of bias assessment in PRISMA guideline and meta-analyzed using inverse variance weight method. Results: Five studies met the inclusion criteria. The number of participants of studies ranged from 58 to 200. The duration of studies ranged from 1 to 3 months. Efficacy in reducing dental plaque was assessed by changes in mean reduction of patient hygiene performance (PHP) and Quigley Hein (Turesky) scores from baseline. Efficacy in reducing gingivitis was assessed by changes in mean reduction of Loe and Silness scores from baseline. Quality assessment of the articles revealed one paper having low bias. Results of the meta-analysis showed the powered toothbrush significantly reduced gingivitis in children more than manual toothbrush (mean difference = 0.04 (95% CI = 0.02, 0.05), P <0.001). However, Efficacy in reducing dental plaque showed no statistical differences between powered and manual toothbrushes (standard mean difference = 1.04 (95% CI = -0.80, 2.88), P=0.27). Conclusion: Powered toothbrush is more effective than manual toothbrush in reducing gingivitis in children. However, plaque removal efficacy showed no statistical differences between powered and manual toothbrushes.
