ต้นทุนการดูแลแบบประคับประคองในโรคมะเร็งระยะลุกลามของโรงพยาบาลตราด

บทคัดย่อ
วัตถุประสงค์: เพื่อวิเคราะห์ต้นทุนการดูแลแบบประคับประคองต่อรายในผู้ป่วยมะเร็งระยะลุกลามในมุมมองสังคม วิธีการ: การวิจัยนี้เป็นการวิจัยเชิงพรรณนา ต้นทุนที่ศึกษาแบ่งเป็น 3 ประเภท คือ ต้นทุนทางตรงที่เกี่ยวกับการแพทย์ ต้นทุนทางตรงที่ไม่เกี่ยวกับทางการแพทย์ และต้นทุนทางอ้อม การคำนวณต้นทุนส่วนของโรงพยาบาลใช้ข้อมูลย้อนหลังจากฐานข้อมูล ส่วนข้อมูลต้นทุนที่ชุมชนต้องจ่ายได้จากการเก็บข้อมูลไปข้างหน้าด้วยการสัมภาษณ์ผู้ป่วยหรือผู้ดูแลตั้งแต่เริ่มการดูแลแบบประคับประคองไปจนครบ 6 เดือน ผลการวิจัย: ต้นทุนทางสังคมรวมทั้งหมดสำหรับผู้ป่วย 35 รายที่ศึกษาเท่ากับ 3,284,265 บาท ต้นทุนต่อผู้ป่วย 1 รายแบ่งเป็นต้นทุนทางตรงทางการแพทย์ 61,277 บาท (ร้อยละ 65.30) ต้นทุนทางตรงที่ไม่เกี่ยวกับทางการแพทย์ 17,671 บาท (ร้อยละ 18.83) และต้นทุนทางอ้อม 14,887 บาท (ร้อยละ 15.87) โดยรวมต้นทุนส่วนของโรงพยาบาลสูงกว่าต้นทุนในชุมชน การรับบริการแบบผู้ป่วยนอกมีต้นทุนเฉลี่ย 245 บาทต่อครั้ง การรับบริการแบบผู้ป่วยในมีต้นทุนเฉลี่ย 2,710 บาทต่อวันนอน และในชุมชนมีต้นทุนเฉลี่ย 221 บาทต่อวัน สรุป: ข้อมูลต้นทุนที่พบในการศึกษานี้สามารถนำไปประกอบการบริหารงบประมาณหรือการช่วยเหลือ การให้ความรู้ และการดูแลผู้ป่วยที่กลับไปรักษาตัวที่บ้านเพื่อเป็นการช่วยลดภาระต้นทุนทั้งของโรงพยาบาลและครอบครัวผู้ป่วยได้
