Cost of Palliative Care in Advanced cancer at Trat Hospital

Abstract
Objective: To analyze the cost of palliative care per case in patients with advanced cancer from a societal perspective. Method: This research was a descriptive study. Costs were divided into 3 types including direct medical costs, direct non-medical cost and indirect cost. Calculation of the hospital's cost was based on retrospective data from the database. Cost incurred in the community was obtained from the prospective data collection by interviewing patients or caregivers from the start of palliative care to the sixth months. Results: Total societal cost for 35 studied patients was 3,284,265 baht. Cost per patient was divided into 61,277 baht (65.30%) for direct medical costs, 17,671 baht (18.83%) for direct non-medical costs and 14,887 baht (15.87%) for indirect costs. Overall hospital cost was higher than the cost incurred in the community. The average cost of outpatient services was 245 baht per visit. Average cost of inpatient services was 2,710 baht per hospital day. Cost incurred in the community was 221 baht per day. Conclusion: Cost information obtained in the study could be used for budget management or provision of assistance, educating and taking care of patients discharged to have home care in order to help reduce costs for both the hospital and the patient's family.
