ประสิทธิภาพของน้ำมันดินในการรักษาโรคสะเก็ดเงิน

บทคัดย่อ
โรคสะเก็ดเงินเป็นโรคทางพันธุกรรมชนิดหนึ่งที่มีภาวะการอักเสบของผิวหนังแบบเรื้อรัง สามารถพบได้ประมาณร้อยละ 2 ของประชากรทั้งหมด ปัจจัยกระตุ้นที่ทำให้อาการของโรคเกิดการกำเริบ เช่น อาหารจำพวกหมักดอง สารเคมี เครื่องดื่มแอลกอฮอล์ ยาบางชนิด หรือโรคที่เกี่ยวข้องกับอวัยวะภายในต่าง ๆ เช่น โรคตับ โรคไต เป็นต้น ตำแหน่งรอยโรคที่มักเกิดผื่นผิวหนังอักเสบของโรคสะเก็ดเงินที่พบได้บ่อยคือผิวหนังบริเวณที่มักมีการเสียดสีหรือแกะเกา เช่น ข้อศอก เข่า แขน ขา ก้นกบ ลำคอ ศีรษะ เป็นต้น ลักษณะสำคัญของผื่นคือ เป็นปื้นหนา มีสะเก็ดสีขาว การรักษาโรคสะเก็ดเงินมีหลายวิธี ได้แก่ การรักษาด้วยยาทาภายนอก การรักษาด้วยยารับประทาน การรักษาด้วยยาฉีดกลุ่มชีวภาพ และการใช้แสงอาทิตย์เทียม แนวทางการรักษาสะเก็ดเงินสามารถทำได้ 2 วิธีคือ 1. การรักษาโดยใช้ยารักษาเดี่ยว หรือ 2.การรักษาด้วยวิธีผสมผสานที่หลากหลาย การรักษาด้วยยาทาภายนอกกลุ่มสเตียรอยด์จัดเป็นทางเลือกแรกในการรักษาโรคสะเก็ดเงินที่มีความรุนแรงระดับน้อยถึงปานกลาง แต่เนื่องจากการรักษาพบผลข้างเคียงจากการใช้ยาในระยะยาวค่อนข้างมาก เช่น ผิวแตกลายถาวร (striae) ปานหรือห้อเลือดบนผิวหนัง (bruising) และเกิดภาวะดื้อยาสเตียรอยด์ ดังนั้นการรักษาโรคสะเก็ดเงินในประเทศไทยจึงนิยมใช้น้ำมันดิน (tar หรือ coal tar) เนื่องจากมีหลักฐานทางวิชาการหลายการศึกษาที่แสดงถึงประสิทธิภาพที่ดีในการรักษาโรคสะเก็ดเงิน โดยทำให้การอักเสบของรอยโรคสงบลงไปได้ยาวนาน และลดโอกาสการกลับเป็นซ้ำใหม่ได้ช้ากว่าเมื่อเทียบกับการใช้ยาทาสเตียรอยด์
