บทคัดย่อ
วัตถุประสงค์: เพื่อทบทวนวรรณกรรมเกี่ยวกับกฎหมาย นโยบาย และมาตรการต่าง ๆ ในการกำกับดูแลการใช้ยานอกข้อบ่งใช้ (off-label use) ในต่างประเทศ วิเคราะห์นโยบายและจัดทำข้อเสนอเชิงนโยบายในการกำกับดูแลการใช้ยานอกข้อบ่งใช้ในประเทศไทย วิธีการ: การศึกษานี้เป็นวิจัยเอกสารโดยสืบค้นเอกสารงานวิจัยจากฐานข้อมูลอิเล็กทรอนิกส์ เช่น Medline และ Google scholar ตั้งแต่เริ่มมีฐานข้อมูลจนถึงเดือนธันวาคม พ.ศ. 2561 ผลการวิจัย: จากการวิจัยเอกสาร 39 ฉบับ พบว่า มาตรการกำกับดูแลการใช้ยานอกข้อบ่งใช้ เพื่อลดความเสี่ยงของผู้ใช้ยามีหลายแนวทาง ได้แก่ การใช้กฎหมาย แนวทางการรักษามาตรฐาน จริยธรรมในการประกอบวิชาชีพ การสร้างแรงจูงใจในการทำวิจัย การใช้กลไกการเบิกจ่ายในระบบประกันสุขภาพ การให้ข้อมูลผู้ป่วย และการพัฒนาระบบการขึ้นทะเบียนยานอกข้อบ่งใช้ มาตรการที่ใช้ทั้งหมดมีความเกี่ยวข้องกับผู้มีส่วนได้ส่วนเสียทุกกลุ่ม ทั้งในกลุ่มหน่วยงานที่กำกับดูแลระบบยา ผู้ผลิตยา สถานพยาบาล องค์กรวิชาชีพด้านการแพทย์ ผู้สั่งใช้ยา และผู้ป่วย ข้อเสนอแนะเชิงนโยบายที่เหมาะสมสำหรับประเทศไทย คือ มาตรการประเมินเทคโนโลยีด้านสุขภาพโดยคัดเลือกยานอกข้อบ่งใช้ที่ส่งผลต่อการเข้าถึงยาของผู้ป่วยภายใต้หลักฐานเชิงประจักษ์ ร่วมกับมาตรการการเบิกจ่ายในระบบประกันสุขภาพ การพัฒนาแนวทางการรักษามาตรฐาน รวมถึงแพทย์ควรให้ข้อมูลแก่ผู้ใช้ยาและให้ผู้ป่วยมีส่วนร่วมในการตัดสินใจในการรักษาด้วยยานอกข้อบ่งใช้ และต้องมีมาตรการในการส่งเสริมให้บริษัทผู้ผลิตทำการวิจัยและขึ้นทะเบียนยาเพิ่มตามข้อบ่งใช้ใหม่ในการวิจัย สรุป: มาตรการการกำกับดูแลการใช้ยานอกข้อบ่งใช้ ประกอบด้วย มาตรการด้านกฎหมาย มาตรการเพิ่มการศึกษาวิจัย มาตรการใช้กลไกการเบิกจ่ายในระบบประกันสุขภาพ มาตรการด้านจริยธรรมของผู้ประกอบวิชาชีพ และมาตรการพัฒนาระบบการขึ้นทะเบียนยา
