ประสิทธิผลและความปลอดภัยของการใช้ยา mecobalamin ในการรักษาภาวะปวดปลายประสาทจากการติดเชื้อไวรัสงูสวัด : การทบทวนวรรณกรรมอย่างเป็นระบบและการวิเคราะห์อภิมานของการทดลองแบบสุ่ม

บทคัดย่อ

วัตถุประสงค์: เพื่อประเมินประสิทธิผลและความปลอดภัยของยา mecobalamin ในการรักษาภาวะปวดปลายประสาทจากการติดเชื้องูสวัด วิธีการ: ผู้วิจัยสืบค้นรายงานการวิจัยที่ตีพิมพ์จากฐานข้อมูลอิเล็กทรอนิกส์ ได้แก่ Pubmed, EMBASE, Scopus, CINAHL, Cochrane Library, Web of Science, www.ClinicalTrials.gov และฐานข้อมูลในไทยตั้งแต่เริ่มต้นฐานข้อมูลจนถึงสิ้นเดือนกุมภาพันธ์ 2561 ผู้วิจัยคัดเลือกงานวิจัยเชิงทดลองแบบสุ่มและมีกลุ่มควบคุมที่ประเมินผลของยา mecobalamin ในผู้ป่วยโรคปวดปลายประสาทจากการติดเชื้องูสวัด ผลลัพธ์หลัก คือ คะแนนความปวด ส่วนผลลัพธ์รองคือ สัดส่วนของผู้ที่มีอาการปวดโดยรวมดีขึ้นมากกว่าร้อยละ 30 (อัตราความสำเร็จ) และการเกิดเหตุการณ์ไม่พึงประสงค์ การวิเคราะห์ทั้งหมดใช้โมเดลการวิเคราะห์แบบสุ่มและแสดงผลด้วยความแตกต่างของค่าเฉลี่ยแบบถ่วงน้ำหนัก (weighted mean difference หรือ WMD) และอัตราเสี่ยงสัมพัทธ์ (relative risk หรือ RR) กับช่วงความเชื่อมั่นร้อยละ 95 (95%CI) การวิเคราะห์หลัก คือ การวิเคราะห์อภิมาน (แบบดั้งเดิม) การวิเคราะห์รองทำโดยการรวมผลลัพธ์แบบการวิเคราะห์อภิมานเครือข่ายตาม frequentist framework และประมาณค่า Surface Under the Cumulative Ranking (SUCRA) ของยาต่าง ๆ โดยใช้สถิติแบบ Bayesian ผลการวิจัย: ผลการสืบค้นพบงานวิจัยที่เกี่ยวข้องทั้งหมด 462 ฉบับ แต่มีงานวิจัยที่ถูกคัดเข้าการศึกษานี้ 5 ฉบับซึ่งศึกษาผลการใช้ยา mecobalamin ทั้งแบบเดี่ยวและแบบผสมนาน 2-4 สัปดาห์ในผู้ป่วยโรคปวดปลายประสาทจากการติดเชื้องูสวัดจำนวน 498 ราย งานวิจัย 3 ใน 5 ฉบับมีความเสี่ยงในการเกิดอคติสูงเมื่อประเมินด้วย Cochrane Risk of Bias (ROB) 2.0 ผลการวิเคราะห์อภิมาน พบว่า การใช้ยาฉีด mecobalamin ลดคะแนนความปวดได้มากกว่าการใช้ยา lidocaine  (WMD=3.11, 95%CI; 2.53-3.70) และมีอัตราความสำเร็จมากกว่าการใช้ยา lidocaine อย่างมีนัยสำคัญทางสถิติ (RR =3.70, 95%CI: 2.22-6.25) ผลการวิเคราะห์อภิมานเครือข่ายพบว่า อัตราความสำเร็จในกลุ่มที่ได้ยาฉีด mecobalamin อย่างเดียวและกลุ่มที่ได้ยา mecobalamin ร่วมการรักษาอื่นสูงกว่ากลุ่มที่ได้ยา lidocaine อย่างมีนัยสำคัญทางสถิติ (RR=3.69, 95%CI: 2.21-6.18 และ RR=4.27, 95%CI: 2.50-7.32 ตามลำดับ) ไม่มีรายงานเหตุการณ์ไม่พึงประสงค์ที่รุนแรง สรุป: หลักฐานในปัจจุบันบ่งชี้ว่า การใช้ยา mecobalamin แบบฉีดอย่างเดียวหรือการใช้ยานี้ร่วมกับการรักษาอื่นน่าจะมีประสิทธิผลดี และมีความปลอดภัยในการรักษาภาวะปวดปลายประสาทจากการติดเชื้องูสวัด อย่างไรก็ตามควรมีการวิจัยเพิ่มเติมในตัวอย่างขนาดใหญ่และมีการออกแบบการวิจัยที่ดีเพื่อสนับสนุนข้อสรุปนี้
