บทคัดย่อ

วัตถุประสงค์: เปรียบเทียบผลของการควบคุมองค์ประกอบทางเมแทบอลิกระหว่างผู้ป่วยกลุ่มอาการเมแทบอลิกที่ได้รับคำแนะนำโดยเภสัชกรร่วมกับการติดตามทางโทรศัพท์และกลุ่มที่ได้รับคำแนะนำโดยเภสัชกรเพียงอย่างเดียว วิธีการ: เป็นการวิจัยเชิงทดลองแบบสุ่มและมีกลุ่มควบคุมในผู้ป่วยกลุ่มอาการเมแทบอลิกที่เข้ารับการรักษาในแผนกผู้ป่วยนอกอายุรกรรม โรงพยาบาลพระนครศรีอยุธยา ระหว่างเดือน มกราคม 2560 ถึง กันยายน 2560 การศึกษาแบ่งผู้ป่วยเป็น 2 กลุ่ม คือ กลุ่มควบคุมซึ่งได้รับคำแนะนำโดยเภสัชกรเพียงอย่างเดียวและกลุ่มทดลองซึ่งได้รับคำแนะนำโดยเภสัชกรร่วมกับการติดตามทางโทรศัพท์ ผู้ป่วยทุกรายได้รับการบริบาลทางเภสัชกรรมและคำแนะนำการปรับเปลี่ยนพฤติกรรมเป็นรายบุคคลโดยผู้วิจัย จากนั้นเฉพาะกลุ่มทดลองได้รับการติดตามทางโทรศัพท์ทุก 2 สัปดาห์ รวมทั้งสิ้น 5 ครั้ง ผลลัพธ์คือค่าขององค์ประกอบทางเมแทบอลิก ได้แก่ เส้นรอบเอว ความดันโลหิต ระดับไตรกลีเซอไรด์ในเลือด ระดับเอชดีแอลในเลือด ระดับน้ำตาลในเลือด ความดันโลหิตในช่วงที่หัวใจบีบตัวและคลายตัว ผลการวิจัย: ผู้ป่วยเข้าร่วมการวิจัย 245 ราย เป็นกลุ่มทดลอง 111 ราย กลุ่มควบคุม 123 ราย เมื่อสิ้นสุดการศึกษา (9-12 สัปดาห์) ผู้ป่วยในกลุ่มทดลองมีค่าเฉลี่ยของระดับน้ำตาลในเลือดน้อยกว่ากลุ่มควบคุมผู้ป่วยอย่างมีนัยสำคัญทางสถิติ (P=0.009) ส่วนองค์ประกอบทางเมแทบอลิกอื่น ๆ ไม่มีความแตกต่างกัน สรุป: การให้คำปรึกษาโดยเภสัชกรร่วมกับการติดตามทางโทรศัพท์ในผู้ป่วยกลุ่มอาการเมแทบอลิกสามารถทำให้องค์ประกอบทางเมแทบอลิกดีขึ้นได้ โดยเฉพาะระดับน้ำตาลในเลือด
