การทดสอบความตรงและความเที่ยงของแบบวัดคุณภาพชีวิต สำหรับผู้ป่วยมะเร็งต่อมไทรอยด์ฉบับภาษาไทย

บทคัดย่อ
วัตถุประสงค์: เพื่อพัฒนาและทดสอบความตรงและความเที่ยงของแบบวัด Quality of Life-Thyroid Version (QOL-TV) ฉบับภาษาไทย วิธีการวิจัย:แบบวัดต้นฉบับภาษาอังกฤษถูกแปลโดยวิธีการแปลแบบ forward-backward แบบวัดต้นฉบับประกอบด้วยคำถาม 56 ข้อ ใน 4 มิติ คือ มิติความสุขสบายทางกาย ใจ สังคม และจิตวิญญาณ กลุ่มตัวอย่างที่ใช้ทดสอบคุณสมบัติของแบบวัดเป็นผู้ป่วยโรคมะเร็งต่อมไทรอยด์ 334 คนในโรงพยาบาลสงขลานครินทร์ การทดสอบความเที่ยงใช้การวิเคราะห์ความสอดคล้องภายใน การวิจัยใช้การวิเคราะห์องค์ประกอบเชิงสำรวจเพื่อค้นหาจำนวนมิติที่เหมาะสมในตัวอย่างชาวไทย การทดสอบความตรงตามเกณฑ์ใช้การหาความสัมพันธ์ระหว่างมิติของแบบวัด QOL-TV กับ FACT-G นอกจากนี้ยังทดสอบความตรงโดยหาความสามารถของแบบวัดในการจำแนก ผลการวิจัย:ความเที่ยงของ QOL-TV ในทุกมิติอยู่ในระดับที่ยอมรับได้ (Cronbach’s alpha > 0.7) การวิเคราะห์องค์ประกอบพบว่า แบบวัด QOL-TV ประกอบด้วย 6 มิติ ได้แก่ ความสุขสบายทางกาย ความสุขสบายทางใจ การปลอดความเครียดจากการรักษา การปลอดความกังวลเรื่องโรคในอนาคต ความสุขสบายทางสังคม และความสุขสบายทางจิตวิญญาณ โดยรวมแล้ว ความสัมพันธ์ระหว่าง QOL-TV กับ FACT-G มีขนาดและทิศทางเป็นไปตามสมมติฐาน ซึ่งบ่งชี้ความตรงตามเกณฑ์ที่เกี่ยวข้องของแบบวัด บางมิติของ QOL-TV สามารถจำแนกแยกความแตกต่างของคุณภาพชีวิตในผู้ป่วยตามคะแนน ECOG การมีโรคร่วม และระยะเวลาการรักษา แต่ FACT-G ไม่สามารถแยกความแตกต่างได้เลย ทั้ง QOL-TV และ FACT-G สามารถจำแนกความแตกต่างของคุณภาพชีวิตตามการได้รับหรือหยุดยาฮอร์โมน และระยะของโรค ขนาดอิทธิพลที่พบก็มีค่าใกล้เคียง มีเพียงการจำแนกความแตกต่างตามอายุผู้ป่วยที่ FACT-G สามารถจำแนกความแตกต่างได้ดีกว่า QOL-TV สรุป: ผลการศึกษาสรุปได้ว่า แบบวัด QOL-TV มีความเที่ยง ความตรง อยู่ในระดับที่น่าพอใจ ผู้ป่วยโรคมะเร็งต่อมไทรอยด์เข้าใจเนื้อความในแบบวัดได้ดี บุคลากรทางการแพทย์สามารถนำแบบวัดไปประเมินผลลัพธ์การรักษา
