The data of perception and self-care behaviors towards prevention of chronic kidney disease can be planned to change the hypertensive patient behaviors. The purpose of this analytical research was to assess the correlation between perception and self-care behaviors towards chronic kidney disease prevention. A study of 186 hypertensive patients undergoing inclusion-exclusion criteria was conducted on June-July, 2020 at Tambon Pasao Health Promoting Hospital, Amphur Mueng, Uttaradit Province. Data were collected using questionnaire created by the researchers and analyzed the data with descriptive and Pearson's correlation statistics. Results of this study showed that the hypertensive patients had a very good average score in the overall perception and a good average score in the overall self-care behaviors towards chronic kidney disease prevention. The overall perception was positively correlated to self-care behaviors towards chronic kidney disease prevention at low level with a statistical significance. In conclusion, the patients who have perceived susceptibility, perceived severity, perceived benefits of taking health action can be better self-care behaviors towards chronic kidney disease prevention. However, other factors should be in depth studied, related to perception and self-care behaviors in hypertensive patients
