Infection of high-risk human papillomavirus (HPV) is an essential cause of cervical cancer. The objectives of this retrospective study were to evaluate the prevalence of high-risk HPV and accuracy between colposcopy results and pathology results of cervical specimens in patients with high-risk HPV infection in Srisangworn Sukhothai Hospital. This study was conducted by reviewed the medical records or electronic health records of patients who received primary HPV screening, colposcopy-directed biopsy, or loop electrosurgical excision procedure (LEEP) between February 2021 to September 2021. There were 1,882 screening cases with a mean age of 48.52 years. Previously given birth had been found in 1,753 cases (93.1%). Prevalence of high-risk HPV infection was 8% (150 cases). Cervical pathology of cervical intraepithelial neoplasia (CIN) 2 or higher with high-risk HPV types 16, 18, 31, 33, 52, 58 were found in five patients. Accuracy between colposcopy result and pathology of directed biopsy and LEEP specimen was 84.6% and 69.0% respectively. Colposcopy results with directed biopsy had sensitivity 66.7% and specificity 100%. Therefore, in high risk HPV infection patients, a cervical endoscopy should be performed promptly to diagnose precancerous lesions and early cervical cancer
