Factors Associated with Dental Service Utilization Among Working-Age Population in Bangrakam Hospital, Bangrakam District, Phitsanulok Province

Abstract

Oral health problems among working-age population remain a major concern that affects both quality of life and work productivity. Promoting regular dental visits is therefore essential for preventing oral diseases.

This study aimed to examine the relationships between personal factors, oral health knowledge, health belief model perceptions, and oral health care behaviors with dental service utilization among working-age population.

Methods: A cross-sectional analytical study was conducted between June–July 2025 among 224 outpatients aged 25–59 years at Bangrakam Hospital, selected by systematic random sampling. Data were collected using a validated questionnaire (KR-20 = 0.744 for knowledge; Cronbach’s alpha=0.879 for health beliefs; 0.776 for behaviors) and analyzed with descriptive and multivariable logistic regression. Results: Participants were mainly female (68.3%), mean age 38.4 years. Nearly half were employees/self-employed (45.9%), and 35.8% earned <10,000 THB/month. Oral health knowledge and health belief perceptions were high, while oral health behaviors were moderate. Over half (50.4%; 95% CI: 43.9.-56.9) had not used dental services in the past year. Factors significantly associated with non-utilization were male gender (OR=1.99; 95% CI: 1.01-3.92), absence of oral pain (OR=2.19; 95% CI: 1.15-4.17), lower perceived susceptibility (OR=0.81; 95% CI: 0.71-0.94), and higher perceived barriers (OR=1.08; 95% CI: 1.01-1.16). Conclusion: Dental service utilization was low. Gender, oral symptoms, perceived susceptibility, and perceived barriers were key factors, suggesting the need for strategies to raise awareness and reduce barriers, especially among men and those without oral symptoms.
