Predictors of Adherence to the Use of Insulin Injection in Patients with Type 2 Diabetes Mellitus in Non-Communicable Diseases Clinic of Cha-am Hospital

Abstract
Objective: To determine the predictors of adherence to the use of insulin injection in patients with type 2 diabetes mellitus. Method: This cross-sectional analytical study was conducted among type 2 diabetes patients receiving insulin therapy in the noncommunicable diseases clinic of Cha-am Hospital. The Medication Adherence Scale for Thai was used to assess medication adherence. Multiple logistic regression analysis was used to determine the predictors of adherence to insulin use and the results were presented as adjusted odds ratio (aOR) and 95% confidence interval (95% CI). Results: 260 patients participated in the study with a mean age of 50 years ± 1.28 years, and 51.2% of them were males. 128 patients (49.23%) showed an adequate adherence to the use of insulin injections. Types of physicians and patient's knowledge on diabetes were significantly associated to adherence in the use of insulin injections. Patients receiving care from an internist showed 1.872 times more adherence to insulin use (OR 1.872, 95%CI 1.120, 3.130) compared to those receiving care from general practitioner. Patients with adequate knowledge on diabetes showed 3.086 times more adherence to insulin use (OR 3.086, 95%CI 1.161, 8.201) compared to those with inadequate knowledge on diabetes. Conclusion: Predictors of adherence to insulin injection were types of physicians and patient’s knowledge on diabetes. The results of this study suggest the importance of patient care provided by internists and the promotion of patient education on diabetes to improve insulin injection adherence in type 2 diabetes patients.
