Development of Multimedia in Northeastern Dialect for Demonstration of the Use Metered Dose Inhaler and their Testing in Asthmatic Patients

Abstract
Objective: To develop multimedia in Northeastern dialect for demonstrating the use of metered dose inhaler (MDI) and to test the effects of using such media in asthmatic patients on clinical outcomes. Method: The study was divided into: 2 phases. Phase 1 was the development of multimedia in Northeastern dialect for demonstrating the use of MDI. The demonstration was divided into 2 parts: exercises on breathing in relation to inhalation and 10 steps of inhaler use techniques. The multimedia was animation with Northeastern folk music as background, audio narration in local dialect, i.e., Northeastern dialect with a central Lao accent and subtitles. Animations were delayed or repeated at key steps to make them easier to understand and remember. Phase 2 was a one-group quasi-experimental study that measured the outcomes before and after intervention with the objective to assess the effects of multimedia in 75 patients. The study assessed the appropriateness of patients’ inhaler use before and immediately after viewing multimedia at Weeks 0 and 4. Measured clinical outcomes were ACT score, FEV1%Predicted, and %FEV1. Results: The mean appropriateness score for inhaler use after multimedia viewing and counseling significantly increased (from 5.61±1.65 to 8.81±1.09, P<0.001), but decreased over time. The score before the second media viewing and counseling at week 4 was lower than that after first media viewing and counseling (from 8.81±1.09 to 8.09±1.76, P<0.001). However, when the participants viewed the media and received the advice for the second time, the mean score increased and was significantly higher than that after the first counseling (from 8.09±1.76 to 8.99±0.98). The result was consistent with the changes of clinical outcomes. At Week 4, mean ACT score, FEV1%Predicted, and %FEV1 significantly increased from Week 0 (from 20.27 to 20.97, P<0.001; from 84.88±11.44 to 90.87±12.21, P<0.001; from 74.37±22.59 to 77.68±20.12, P=0.003, respectively). Moreover, asthma control was significantly improved from 52 (69.33 percent) to 62 (82.67 percent) cases (P< 0.001). Conclusion: The developed multimedia in conjunction with counseling could improve the appropriateness of MDI use and could also improve clinical outcomes in asthmatic patients who communicate in Northeastern dialect.
