Effect of Village Health Volunteers Based-Smoking Cessation Program on Smoking Behavior of Monks at Phachi District, Phra Nakon Sri Ayutthaya Province

Abstract
Objective: To compare the outcomes of smoking cessation program for monks between that with and without the participation of village health volunteers (VHVs) at Phachi District, Phra Nakon Sri Ayutthaya Province. Method: This study was an experimental trial with the two groups and pre-post test. VHVs in the program attended the training on knowledge regarding the help for smoking cessation and operating procedures from pharmacists before implementation of the program. The subjects consisted of 66 monks in 8 sub-districts who smoked regularly. The researchers randomly allocated sub-districts into control and experimental groups with 33 monks in each group. The experimental group received smoking cessation program which included weekly follow up by VHVs. Duration of study was 12 weeks. The study collected the data on daily smoking, smoking behavior and severity of nicotine dependence using Fagerstrom test. Results: When evaluated from 7-day point prevalence abstinence, experimental group had a higher rate of abstinence than control group at the 4th week (6 monks (18.2%) and 1 monk (3%), respectively, P=0.105) and at the 12th week (21 monks (63.6 %) and 7 monks (21.2%), respectively, P=0.001). However, no monks had a continuous abstinence at the 4th and 12th week. The means of smoking behavior in the experimental group decreased more than the reduction in control group for 8.85±2.01 points (P<0.001). The means of smoking behavior decreased from 25.52±4.47 to 12.97±7.62 (P<0.001) and from 26.58±4.64 to 21.82±8.73 (P=0.002) in the experimental group and the control group respectively. Nicotine dependence in the experimental group decreased more than that in the control group for 2.61±0.61 points (P<0.001). The means of nicotine dependence decreased from 4.61±1.66 to 1.36±2.19 (P<0.001) and from 5.06±1.83 to 3.97±2.73 (P=0.003) in the experimental group and the control group respectively. Conclusion: Smoking cessation program with and without the participation of VHVs in monitoring reduced dependence level and smoking behavior of monks. However, the effect on smoking cessation at the 12th week in the program with VHVs participation was significantly higher than that in the program without VHVs. Therefore the smoking cessation program for monks should be supported and VHVs should be encouraged to participate in the program.
