Reasons for Medication Non-adherence from the Perspective of Type 2 Diabetic Patients: a Case Study of Warit-Chaphum Hospital, Sakon Nakorn

Abstract
Objective: To study the reasons of medication non-adherence from the perspective of type 2 diabetic patients. Method: This study was a qualitative study collecting the data by in-depth interview with 16 diabetic patients receiving medication treatment at Warit-Chaphum Hospital, Sakon Nakhon during May 2017 - December 2018. Data analysis was done by content analysis. Results: Three out of 16 key informants were medication adherent patients, while 13 key informants were medication non-adherent by reducing the dose, using herbal drugs concomitantly with medications prescribed by physicians, and not taking certain drugs. The main reasons leading key informants to become medication non-adherent were the many drugs items to be taken, complicated instruction for taking medications, occurrence of adverse reactions from medications or having negative experience with medications, hectic schedules or forgetfulness, and positive attitude towards herbal medicines. In addition, key informants had models explaining illnesses of diabetes that were different from those of health professionals. As a result, they modified drug taking behaviors according to ones’ own concepts. Conclusion: Key informants' beliefs and perceptions greatly affected medication adherence. Health personnel should understand the phenomena that are important factors affecting treatment outcomes, and develop intervention for increasing medication adherence.
