Outcomes of Pharmaceutical Care in Outpatients with Alzheimer’s Disease: A Pilot Study

Abstract
Objectives: To examine the effects of pharmaceutical care on drug related problems (DRPs) in patients with dementia and knowledge on drugs and the disease, quality of life (QoL), and caregivers’ perceived burden in taking care of patients. Methods: This study was a quasi-experimental research in one group of subjects with pretest and posttest. The subjects were patients with dementia and their caregivers, 12 in each group, receiving care from outpatient department at the Lelawadee clinic at Mahasarakham Hospital between August 1 to December 31, 2016. The researchers identified and resolved DRPs in each patient for 3 times at the baseline (day 1), 30 and 90 days after intervention. The researchers also counseled the caregivers on the disease, drug and basic caregiving, and provided them with a handbook on taking care of patients with dementia. Results: The majority of dementia patients were females (58.3%) with mean age of 72.9±12.7 years. Comparison of the data at baseline and 3 months after intervention revealed that the average number of DRPs per patients significantly decreased from 0.83±0.72 to 0.25±0.45 (P=0.038). DRPs were completely resolved (100%) were medication non-adherence, not receiving needed
medications, and sub-therapeutic dosage. DRP with 50% decrease after intervention were adverse drug reaction. Most caregivers were female (75%) with an average age of 46.2±9.0 years old and being patients’ offspring (66.7%). Comparison of the data at baseline and after receiving education and handbook from the researchers revealed that knowledge scores on the disease and drugs significantly increased (9.1±3.6 vs 12.6±3.7 from the full score of 20, P=0.001). QoL and perceived burden of caregivers tended to improve but were not significant. Conclusion: Pharmaceutical care provision could decrease the number of DRPs in people with dementia. Educating about drugs and the disease with the distribution of handbooks significantly improves caregivers’ knowledge.
