Prevalence and Characteristics of Symptomatic Orthostatic Hypotension in Thai Patients with Parkinson’s Disease

Abstract
Objective: To determine prevalence and characteristics of symptomatic orthostatic hypotension in Thai patients with Parkinson’s disease (PD). Methods: One-hundred PD patients were enrolled into study during May and August 2015 at Chulalongkorn Center of Excellence for Parkinson’s Disease and Related Disorders, King Chulalongkorn Memorial Hospital. Sitting or supine and standing blood pressures (BP) were measured and the presence of orthostatic hypotension (OH) symptoms in the past week was evaluated by using OHQ (Orthostatic Hypotension Questionnaire). Orthostatic hypotension was defined as systolic BP (SBP) and/or diastolic BP (DBP) falls ≥ 20 and/or ≥ 10 mmHg after standing within 3 minutes. Symptomatic OH was defined as those having BP fall according to manometric definition and had OHQ score > 0. Results: The average age of the patients was 65.5 ±11.6 (mean ± SD) years. Patients were categorized according to the postural BP change and OHQ score into 4 groups, including 1) Symptomatic OH patients; BP fall met OH definition and had OH symptoms (18%) 2) Non-OH patients; BP fall did not meet OH definition and no OH symptoms (36%) 3) Patients having BP fall met OH definition but did not have OH symptoms (4%) and 4) Patients having BP fall did not meet OH definition but had symptoms of OH (42%). The prevalence of OH and symptomatic OH were 22% (95% CI: 14%-30%) and 18% (95% CI: 10%-26%), respectively. The clinical symptoms of OH found in this study included dizziness, lightheadedness, syncope, visual disturbance, weakness, fatigue and head/neck discomfort. Conclusion: OH can be found in PD patients. Some of the OH patients had the clinical symptoms of OH. Blood pressure monitoring and OHQ assessment can help detect the occurrence of OH and symptomatic OH.
