การพัฒนาระบบบริบาลทางเภสัชกรรมที่ผสานการตรวจยีน NAT2 และ การเปลี่ยนแปลงของภาวะตับอักเสบจากยาต้านวัณโรคในโรงพยาบาลชุมชน

บทคัดย่อ

วัตถุประสงค์: เพื่อพัฒนาระบบบริบาลทางเภสัชกรรมที่ผสานผลตรวจยีน NAT2 (N-acetyltransferase 2) สำหรับผู้ป่วยวัณโรคปอดรายใหม่ในโรงพยาบาลชุมชน และประเมินการเปลี่ยนแปลงของภาวะตับอักเสบจากยาต้านวัณโรคภายหลังการนำระบบดังกล่าวไปใช้ วิธีการ: การศึกษานี้เป็นการวิจัยกึ่งทดลองเชิงพัฒนาระบบ กลุ่มตัวอย่างเป็นผู้ป่วยวัณโรคปอดรายใหม่อายุ 18 ปีขึ้นไปที่เริ่มรักษาระหว่างวันที่ 1 ตุลาคม 2566 ถึง 31 ธันวาคม 2568 และได้รับสูตรยามาตรฐาน 2HRZE/4HR การศึกษาคัดออกผู้ติดเชื้อ HIV ผู้มีโรคตับเดิมหรือมีประวัติไวรัสตับอักเสบที่ทราบมาก่อน ผู้มีค่า alanine aminotransferase (ALT) ก่อนเริ่มยามากกว่า 2 เท่าของค่าปกติส่วนบน และผู้ป่วยวัณโรคดื้อยา การพัฒนาระบบบริบาลทางเภสัชกรรมที่ผสานผลตรวจยีน NAT2 ทำภายใต้แนวคิดการบริบาลทางเภสัชกรรม กรอบโครงสร้าง–กระบวนการ–ผลลัพธ์ของ Donabedian และวงจรวางแผน–ปฏิบัติ–ตรวจสอบ–ปรับปรุง (Plan–Do–Check–Act: PDCA) จำนวน 2 รอบ ช่วงก่อนใช้ระบบเต็มรูปแบบคือวันที่ 1 ตุลาคม 2566 ถึง 30 กันยายน 2567 ช่วงเปลี่ยนผ่านคือเดือนตุลาคมถึงธันวาคม 2567 และช่วงหลังใช้ระบบเต็มรูปแบบคือวันที่ 1 มกราคม ถึง 31 ธันวาคม 2568 ข้อมูลช่วงเปลี่ยนผ่านไม่รวมอยู่ในการวิเคราะห์หลัก ผลลัพธ์หลักคืออุบัติการณ์รายเดือนของภาวะตับอักเสบจากยาต้านวัณโรค การศึกษาใช้การวิเคราะห์อนุกรมเวลาแบบรายเดือนที่มีการแทรกแซง (interrupted time series: ITS) แบบจำลองหลักใช้ segmented Poisson regression โดยใช้จำนวนผู้ป่วยวัณโรคปอดรายใหม่รายเดือนเป็นตัวแปรชดเชย ผลการวิจัย: ภายหลังใช้ระบบเต็มรูปแบบ ผู้ป่วยทุกรายได้รับการตรวจ NAT2 และผลตรวจถูกนำมาใช้พิจารณาขนาดยา isoniazid รายบุคคล ผู้ป่วยกลุ่ม rapid และ slow acetylators ได้รับการพิจารณาปรับขนาดยาครบทุกราย ส่วนกลุ่ม intermediate acetylators ได้รับขนาดยามาตรฐานตามแนวทางการควบคุมวัณโรคของประเทศไทย ความสม่ำเสมอในการใช้ยาที่ประเมินจากการนับเม็ดยาเพิ่มขึ้นจากร้อยละ 96.7 ± 1.4 เป็นร้อยละ 97.8 ± 0.9 (P<0.001) ภาวะตับอักเสบจากยาต้านวัณโรคลดลงจาก 60 รายในผู้ป่วย 152 ราย (ร้อยละ 39.5) เป็น 13 รายในผู้ป่วย 165 ราย (ร้อยละ 7.9) การวิเคราะห์ด้วย ITS พบการลดลงอย่างมีนัยสำคัญของอัตราการเกิดภาวะตับอักเสบทันทีหลังใช้ระบบเต็มรูปแบบ (incidence rate ratio หรือ IRR 0.503, 95% CI 0.345–0.732, P<0.001) และพบการลดลงของแนวโน้มรายเดือนหลังใช้ระบบเมื่อเทียบกับช่วงก่อนใช้ระบบ (IRR 0.837, 95% CI 0.747–0.938, P=0.002) สรุป: ภายหลังการนำระบบบริบาลทางเภสัชกรรมที่ผสานผลตรวจ NAT2 ไปใช้ อุบัติการณ์ภาวะตับอักเสบจากยาต้านวัณโรคมีแนวโน้มลดลง พร้อมกับการเพิ่มขึ้นของการตรวจ NAT2 การพิจารณาขนาดยา และการติดตามความปลอดภัยและความสม่ำเสมอในการใช้ยา อย่างไรก็ตาม ผลการศึกษาควรตีความว่าเป็นการเปลี่ยนแปลงที่สัมพันธ์กับการพัฒนาระบบบริการแบบบูรณาการ ไม่ใช่ผลของการตรวจ NAT2 หรือการปรับขนาดยา isoniazid เพียงองค์ประกอบเดียว ผลการศึกษายังไม่สามารถสรุปผลด้านประสิทธิผลทางจุลชีววิทยาหรือผลลัพธ์การรักษาระยะยาวได้
