การส่งมอบยาและการให้คำแนะนำในการใช้ยาในหน่วยบริการปฐมภูมิของเขตสุขภาพที่ 10

บทคัดย่อ
วัตถุประสงค์: เพื่อสำรวจสถานการณ์และวิเคราะห์ปัจจัยที่ส่งผลต่อการส่งมอบยาและการให้คำแนะนำในการใช้ยาในหน่วยบริการปฐมภูมิของเขตสุขภาพที่ 10 วิธีการ: การศึกษาครั้งนี้เป็นงานวิจัยแบบผสมผสาน โดยการสำรวจผู้ส่งมอบยาและให้คำแนะนำในการใช้ยาประจำโรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) ใช้แบบสำรวจประกอบด้วยคำถามเกี่ยวกับการทำกิจกรรม 55 รายการ แบ่งเป็น 6 หมวด ได้แก่ การบริหารจัดการระบบยา (22 ข้อ) มาตรฐานการส่งมอบยาและการให้คำแนะนำในการใช้ยา (10 ข้อ) การส่งเสริมการใช้ยาอย่างสมเหตุผล (9 ข้อ) ระบบเฝ้าระวังความคลาดเคลื่อนทางยา (8 ข้อ) ระบบเฝ้าระวังการแพ้ยา (3 ข้อ) และระบบเฝ้าระวังการใช้ยาความเสี่ยงสูง (3 ข้อ) คำถามเป็นแบบปลายปิดมี 3 ตัวเลือก (มี/ไม่มี/ไม่ทราบ) คะแนนรวมของการปฏิบัติเท่ากับ 55 คะแนน และมีการสัมภาษณ์แบบกึ่งโครงสร้างในผู้ตอบแบบสอบถามจำนวน 9 คนเพื่อค้นหาปัจจัยที่ส่งผลต่อการดำเนินงาน ผลการศึกษา: ผู้ตอบแบบสอบถามมีจำนวน 132 คน ส่วนใหญ่พยาบาลวิชาชีพเป็นผู้ส่งมอบยาและให้คำแนะนำในการใช้ยา (ร้อยละ 93.18) การส่งมอบยาและการให้คำแนะนำในการใช้ยาในภาพรวมมีคะแนนเฉลี่ยเท่ากับ 48.50±8.61 คะแนน (เต็ม 55 คะแนน) คะแนนแยกตามหมวด คือ การบริหารจัดการระบบยา 17.71±5.80 คะแนน (เต็ม 22 คะแนน) มาตรฐานการส่งมอบยาและการให้คำแนะนำในการใช้ยา 9.53±1.03 คะแนน (เต็ม 10 คะแนน) การส่งเสริมการใช้ยาอย่างสมเหตุผล 8.57±1.11 คะแนน (เต็ม 9 คะแนน) ระบบการติดตามความคลาดเคลื่อนทางยา 7.22±1.32 คะแนน (เต็ม 8 คะแนน) ระบบการเฝ้าระวังการแพ้ยา 2.91±0.34 คะแนน (เต็ม 3 คะแนน) และระบบเฝ้าระวังการใช้ยาความเสี่ยงสูง 2.91±0.34 คะแนน (เต็ม 3 คะแนน) การได้รับการฝึกอบรมด้านการส่งมอบยาและการให้คำแนะนำในการใช้ยาและการมีเครื่องพิมพ์ฉลากยา (P<0.001) ช่วยสนับสนุนการทำงานด้านนี้ ผลการสัมภาษณ์พบว่า ปัจจัยที่ส่งผลต่อการดำเนินงานทั้งแง่บวกและลบ ได้แก่ บุคลากร กระบวนการจ่ายยา และการพัฒนาเข้าสู่มาตรฐาน รพ.สต. ติดดาว สรุป: การดำเนินงานการส่งมอบยาและการให้คำแนะนำในการใช้ยาของหน่วยบริการปฐมภูมิของเขตสุขภาพที่ 10 นั้นทำได้ดีในภาพรวม แต่หน่วยบริการปฐมภูมิยังคงต้องการเภสัชกรในการช่วยสนับสนุนการดำเนินงานด้านนี้ จึงควรสนับสนุนให้เภสัชกรได้ปฏิบัติหน้าที่ด้านนี้ในหน่วยบริการปฐมภูมิอย่างสม่ำเสมอ
