การสำรวจโปรแกรมการใช้ยาต้านจุลชีพอย่างสมเหตุสมผลในโรงพยาบาลเขตสุขภาพที่ 6

บทคัดย่อ
วัตถุประสงค์: เพื่อสำรวจกลยุทธ์ ปัญหา และการวัดผลลัพธ์ของการดำเนินการโปรแกรมควบคุมกำกับดูแลการใช้ยาต้านจุลชีพอย่างเหมาะสม (Antimicrobial Stewardship Program; ASP) ของโรงพยาบาลในเขตสุขภาพที่ 6 วิธีการ: การวิจัยเชิงสำรวจครั้งนี้ทำในกลุ่มตัวอย่างเภสัชกรในโรงพยาบาลรัฐบาลและเอกชนในเขตสุขภาพที่ 6 จำนวน 124 แห่ง การศึกษาใช้แบบสอบถามเก็บข้อมูลการควบคุมกำกับดูแลการใช้ยาต้านจุลชีพอย่างเหมาะสมด้านกลยุทธ์ ปัญหาการดำเนินการ และการติดตามและประเมินผล การศึกษาดำเนินการระหว่างวันที่ 30 พฤศจิกายน 2563 – 31 ธันวาคม 2563 ผลการวิจัย: ตัวอย่างในโรงพยาบาล 41 แห่งตอบแบบสำรวจ (อัตราการตอบกลับ ร้อยละ 33.1) โรงพยาบาลทุกแห่งดำเนินการ ASP ตั้งแต่ 3 กลยุทธ์ขึ้นไป โดยกลยุทธ์ที่พบมากที่สุด คือ การทบทวนคำสั่งยาและให้คำแนะนำเพิ่มเติม (ร้อยละ 97.4) รองลงมาได้แก่ การจำกัดการใช้ยาและการขออนุมัติการใช้ยาต้านจุลชีพ (ร้อยละ 86.8) ปัญหาของการดำเนินการ ASP ที่พบโดยส่วนใหญ่ คือ ภาระงานมากและการไม่ปฏิบัติตามเกณฑ์หรือข้อกำหนด โรงพยาบาลร้อยละ 93.6 ที่เข้าร่วมการศึกษาใช้ตัวชี้วัดปริมาณการใช้ยาต้านจุลชีพในหน่วย defined daily dose สรุป: โรงพยาบาลส่วนใหญ่ในเขตสุขภาพที่ 6 มีการดำเนินการ ASP อย่างเหมาะสม โดยทุกแห่งดำเนินการตั้งแต่ 3 กลยุทธ์ขึ้นไป จากปัญหาการดำเนินการที่พบ ทีมงานผู้รับผิดชอบ ASP ควรกำหนดเกณฑ์ให้ชัดเจนเกี่ยวกับการดำเนินการในกลยุทธ์ต่าง ๆ
