อุบัติการณ์ของการพบแบคทีเรียแกรมลบดื้อยาหลายขนานในสิ่งส่งตรวจ ที่เป็นเลือดในโรงพยาบาลชุมชนแห่งหนึ่งในภาคตะวันออกเฉียงเหนือ

บทคัดย่อ

วัตถุประสงค์: เพื่อศึกษาอุบัติการณ์ของการพบแบคทีเรียแกรมลบดื้อยาหลายขนาน (multidrug resistant gram negative bacteria: MDR-GNB) ในสิ่งส่งตรวจที่เป็นเลือด และศึกษารูปแบบความไวต่อยาปฏิชีวนะในโรงพยาบาลชุมชนแห่งหนึ่งในภาคตะวันออกเฉียงเหนือ วิธีการ: การวิจัยนี้เป็นการศึกษาแบบย้อนหลังเชิงพรรณนา โดยใช้ข้อมูลผลเพาะเชื้อแบคทีเรียในสิ่งส่งตรวจที่เป็นเลือดและผลการทดสอบความไวต่อยาปฏิชีวนะย้อนหลัง 5 ปีจาก มกราคม 2559 - ธันวาคม 2563 ผลการวิจัย: อุบัติการณ์ของการพบ MDR-GNB คือ ร้อยละ 28.47 ระหว่าง พ.ศ. 2559 – 2563 อุบัติการณ์รายปี คือ ร้อยละ 24.83, 21.64, 22.07, 38.08 และ 30.86 ของไอโซเลทที่พบ GNB ตามลำดับ เชื้อที่พบมากที่สุดคือ Escherichia coli (E. coli) (ร้อยละ 34.85 ของไอโซเลทที่พบ GNB) รองลงมาเป็น Klebsiella pneumoniae (ร้อยละ 14.19 ของไอโซเลทที่พบ GNB) และ Burkholderia pseudomallei (ร้อยละ 3.19 ของไอโซเลทที่พบ GNB)ตามลำดับ อุบัติการณ์ของการพบเชื้อ MDR-GNB ส่วนใหญ่มีแนวโน้มเพิ่มขึ้น ผู้ป่วยที่พบเชื้อดังกล่าวเป็นเพศชายร้อยละ 52.54 มีอายุตั้งแต่ 60 ปีขึ้นไป (ร้อยละ 57.97) และพบมากที่สุดในหอผู้ป่วยหญิง (ร้อยละ 41.02) ยา ceftriaxone เป็นยาต้านจุลชีพที่มีปริมาณการใช้มากที่สุดในโรงพยาบาล เมื่อจำแนกเชื้อตามการดื้อต่อยาปฏิชีวนะพบว่า เชื้อ E. coli ดื้อต่อยากลุ่ม penicillin มากที่สุด คือ ร้อยละ 98.97 สรุป: อุบัติการณ์ของการพบเชื้อ MDR-GNB มีแนวโน้มเพิ่มสูงขึ้น มีการใช้ยาปฏิชีวนะเพิ่มมากขึ้น และมีอัตราการดื้อยาที่สูงขึ้นในสิ่งส่งตรวจที่เป็นเลือด ดังนั้นโรงพยาบาลซึ่งเป็นสถานที่วิจัยควรมีการจัดการข้อมูลเชื้อดื้อยาอย่างเป็นระบบ โดยควรมีการเฝ้าระวังเชื้อดื้อยาอย่างต่อเนื่อง และควรส่งเสริมให้มีการดำเนินงานระบบสนับสนุนการใช้ยาต้านจุลชีพสมเหตุผล (antimicrobial stewardship program)
