บทคัดย่อ

วัตถุประสงค์:เพื่ออธิบายการคงอยู่ในระบบของชายที่มีเพศสัมพันธ์กับชาย (men having sex with men: MSM) โดยที่ตนมีผลการตรวจ HIV เป็นลบ วิธีการ: การศึกษาคัดเลือกผู้ให้ข้อมูลหลัก16 คนที่เป็น MSM โดยที่ตนเองมีผลการตรวจ HIV เป็นลบที่คงอยู่ในระบบ และเข้ามารับบริการตรวจ HIV ที่กลุ่มองค์กรอิสระ MSM  การเก็บข้อมูลใช้การสัมภาษณ์แบบเจาะลึกรายบุคคลตามแบบสัมภาษณ์ชนิดกึ่งโครงสร้างและชนิดไม่มีโครงสร้าง และการสังเกต การวิเคราะห์ข้อมูลใช้การวิเคราะห์เนื้อหา ผลการวิจัย: การคงอยู่ในระบบสามารถอธิบายจาก 1) การรับรู้ความเสี่ยงการติดเชื้อ HIV จากการไม่ใช้ถุงยางอนามัย การมีอาชีพบริการทางเพศ และการมีคู่นอนหลายคน 2) การรับรู้ความรุนแรงจากผลกระทบทางร่างกายและสังคมของโรค 3) การรับรู้ประโยชน์ของการตรวจ HIV ซ้ำ คือ ได้ทราบผลการติดเชื้อ ทำให้เกิดความสบายใจ เกิดตระหนักในพฤติกรรมการป้องกัน เป็นหลักฐานว่าไม่ติดเชื้อซึ่งทำให้รอดพ้นจากการถูกตีตรา และหากพบว่าติดเชื้อจะทำให้เข้าสู่กระบวนการรักษาได้เร็ว 4) การรับรู้อุปสรรคคือ เวลาการให้บริการของกลุ่มองค์กรอิสระ MSM  สิ่งที่ช่วยลดอุปสรรคคือ ความสะดวกในการเข้ารับบริการ การทราบผลการตรวจได้ภายในวันเดียว ความเป็นส่วนตัวของสถานที่ การตรวจฟรี และความมั่นใจในการเก็บความลับของกลุ่มองค์กรอิสระ MSM สรุป:การคงอยู่ในระบบของ MSM สามารถอธิบายได้ด้วยเงื่อนไขที่พบในการวิจัย ซึ่งจะเป็นประโยชน์ในการวางแผนให้กลุ่มเป้าหมายคงอยู่ในระบบ
