ความคิดเห็นของเภสัชกรชุมชนในกรุงเทพมหานครต่อช่องทางและเงื่อนไข ในการจ่ายยาปฏิชีวนะสำหรับใช้ภายนอก

บทคัดย่อ
วัตถุประสงค์: เพื่อสำรวจความคิดเห็นของเภสัชกรชุมชนในเขตกรุงเทพมหานครต่อช่องทางและเงื่อนไขในการจ่ายยาปฏิชีวนะสำหรับใช้ภายนอก และเพื่อสำรวจความคิดเห็นต่อการส่งเสริมการใช้ยาปฏิชีวนะอย่างสมเหตุผลของร้านยาคุณภาพและร้านยาทั่วไป วิธีการ: แบบสอบถามเกี่ยวกับความคิดเห็นต่อช่องทางและเงื่อนไขในการจ่ายยาปฏิชีวนะสำหรับใช้ภายนอกที่เหมาะสมและความคิดเห็นต่อการส่งเสริมการใช้ยาปฏิชีวนะอย่างสมเหตุผล ถูกออกแบบและส่งไปยังกลุ่มตัวอย่างทางไปรษณีย์ ตัวอย่างในการวิจัย คือ เภสัชกรผู้มีหน้าที่ปฏิบัติการในร้านยาแผนปัจจุบัน ในเขตกรุงเทพมหานคร โดยแบ่งเป็น 2 กลุ่ม คือ ร้านยาคุณภาพ 354 ร้าน และ ร้านยาทั่วไป 915 ร้าน ซึ่งเลือกตัวอย่างแบบอาศัยหลักความน่าจะเป็นโดยใช้เทคนิคแบบชั้นภูมิ ข้อมูลถูกรวบรวมระหว่าง มกราคม ถึง กุมภาพันธ์ พ.ศ. 2560 ผลการวิจัย: ยาปฏิชีวนะสำหรับใช้ทาภายนอก 9 กลุ่มที่ศึกษา เภสัชกรชุมชนร้อยละ 85-95 (ขึ้นกับชนิดของกลุ่มยา) เห็นด้วยกับการอนุญาตให้จ่ายยาดังกล่าวได้ในโรงพยาบาล (ช่องทาง) โดยแพทย์เฉพาะทาง (เงื่อนไข) ตัวอย่างร้อยละ 70-85, 60-80, 10-20 และ 60-80 เห็นด้วยต่อการกำหนดให้สามารถจ่ายได้โดยแพทย์ทั่วไปในโรงพยาบาล แพทย์ประจำคลินิก เภสัชกรร้านยาโดยต้องจัดทำบัญชีขายยา และเภสัชกรร้านยาโดยไม่ต้องจัดทำบัญชีขายยา ตามลำดับ ในขณะที่ยาปฏิชีวนะเฉพาะที่สำหรับตาและหู 6 กลุ่มที่ศึกษา เภสัชกรชุมชนร้อยละ 85-95, 60-80, 50-70, 11-22, และ 50-70 เห็นด้วยต่อการกำหนดให้สามารถจ่ายได้โดยแพทย์เฉพาะทางในโรงพยาบาล แพทย์ทั่วไปในโรงพยาบาล แพทย์ประจำคลินิก เภสัชกรร้านยาโดยต้องจัดทำบัญชีขายยา และเภสัชกรร้านยาโดยไม่ต้องจัดทำบัญชีขายยา ตามลำดับ เภสัชกรชุมชนส่วนใหญ่เห็นด้วยกับแนวคิดต่างๆ ของการส่งเสริมการใช้การใช้ยาปฏิชีวนะอย่างสมเหตุผล แต่ในการปรับสถานะยาปฏิชีวนะจากยาอันตรายเป็นยาควบคุมพิเศษ (ต้องจ่ายตามใบสั่งแพทย์) นั้น เภสัชกรร้านยาคุณภาพมีคะแนนเฉลี่ยของความเห็นด้วย คือ 2.55 (จากคะแนนเต็ม 5 คะแนน) ส่วนร้านยาทั่วไปมีคะแนนเฉลี่ย 2.22 (จากคะแนนเต็ม 5 คะแนน) ซึ่งอยู่ในระดับค่อนข้างไม่เห็นด้วย สรุป: เภสัชกรชุมชนส่วนใหญ่ไม่เห็นด้วยกับการควบคุมการจำหน่ายยาปฏิชีวนะโดยร้านยาและทำบัญชีขายยา ทั้งยังไม่เห็นด้วยกับแนวคิดการปรับสถานะยาปฏิชีวนะเป็นยาควบคุมพิเศษเพื่อส่งเสริมการใช้ยาปฏิชีวนะอย่างสมเหตุผล มาตรการข้างต้นควรได้รับการทบทวน
