Abstract

Sepsis-associated acute kidney injury (SA-AKI) is a common complication among hospitalized patients. It is associated with increased mortality and a higher long-term risk of chronic kidney disease. Prospective data from secondary-care hospitals remain limited. This prospective cohort study aimed to determine renal recovery rates at 7, 30 and 90 days and to identify predictors of 90-day renal recovery in patients with SA-AKI. A total of 166 patients admitted to Langsuan Hospital between June 10 and October 31, 2025 were included. Renal recovery rates at 7, 30 and 90 days were 42.3%, 63.5% and 74.4%, respectively, with most recoveries occurring within the first 30 days in two-thirds of cases. Higher serum albumin levels were independently associated with an increased likelihood of recovery [adjusted hazard ratio (aHR) 1.56, 95% confidence interval (95% CI) 1.12-2.18], whereas longer intensive care unit (ICU) length of stay was associated with a reduced likelihood of recovery (aHR 0.95, 95% CI 0.90-0.99). In conclusion, in patients with SA-AKI, blood albumin levels and length of stay in the intensive care unit may be used as factors to screen for risk and plan future patient follow-up
