Prevalence and Clinical Manifestations of Electrical Injuries, Burns, Scalds, and Agricultural Chemical Injuries among Fruit Farmer Inpatients at Phrapokklao Hospital

Abstract

Injuries from electric shocks, burns, scalding, and agricultural chemical exposure present significant physical and economic losses, particularly among farmers. This study evaluated the prevalence of these injuries among fruit farmers in Chanthaburi Province by analyzing inpatient medical records from Prapokklao Hospital between January 1, 2015 and December 31, 2024. Nine patients were excluded due to incomplete records. Factors associated with in-hospital mortality were analyzed using univariate analysis, reporting crude odds ratios (OR), 95% confidence intervals (CI), and p-values. Among 74 patients analyzed, 64 were male (86.5%), with a median age of 32 years. Causes included electric shock (47; 63.5%), burns (23; 31%), scalding (2; 2.7%), and chemical exposure (2; 2.7%). Compartment syndrome occurred in 8 patients (10.8%), rhabdomyolysis in 5 (6.8%), and in-hospital mortality was 8.1% (6 patients). Univariate analysis revealed statistically significant predictors of mortality: ICU admission (crude OR = 25.91, 95% CI: 2.71-247.78), sepsis (crude OR = 13.40, 95% CI: 0.72-248.80), respiratory failure (crude OR = 67.00, 95% CI: 5.48-819.34), and shock within the first 24 hours (crude OR = 22.83, 95% CI: 1.13-461.22)
