Abstract

Open-heart surgery is a complex and high-risk procedure prone to complications. This research and development  study aimed to develop and evaluate an anesthesia nursing  model in patients undergoing open-heart surgery between  January and October 2025. The study included situation  analysis, design and development, pilot testing and  improvement, and evaluation. Participants included 20 nurse anesthetists and 44 patients, divided into pre- and post-implementation groups of 22 patients each. The research resulted in the "PAC Model," which comprises: 1) practice guidelines of nursing, 2) anesthetist specific competency nurse, and 3) care management of work  assignment. The results showed that after the  implementation of the model, in patients, the incident of  atrial fibrillation and hypothermia decreased significantly (p = 0.012, p < 0.001), while the incident of acute renal failure  tended to decrease but was not statistically significant (p = 0.68). The length of stay in the intensive care unit was significantly reduced (p < 0.001). Regarding nurses, after  using the model the average scores for knowledge and  nursing practice skills were significantly higher than before  using the model (p < 0.001, p < 0.001), with the highest  satisfaction level. The PAC Model can be implemented in  anesthesia nursing care for patients undergoing open-heart surgery to enhance patient safety
