Abstract

Parents increasingly accompany their children during dental care. Clear instructions that define the parent's role as a silent observer may improve cooperation, reduce chairside conflict, and support dentist-child rapport. This randomized, forward-looking, controlled trial aimed to compare compliance with verbal and written instructions as observers among 64 parents who brought their healthy (ASA class I or II) children aged 4-9 years for dental treatment under local anesthesia. Parents were randomly assigned to receive either verbal guidance delivered chairside or a written instruction sheet. Baseline characteristics were collected using questionnaires. During treatment, a trained dental assistant recorded any disruptive behaviors by parents or children, documenting both the number of interruptions and the total duration of interruptions. Categorical variables were analyzed using the exact probability test, Student's t-test and the Wilcoxon rank-sum test. Parents in the verbal-instruction group demonstrated significantly higher compliance than those in the written-instruction group. Furthermore, the study found a correlation between parental age and compliance, with older parents showing a statistically significant tendency to be less cooperative than younger parents. Overall, clear verbal guidance appears to enhance parental compliance during pediatric dental procedures and may be a practical strategy to optimize the treatment environment.
