Preventing Atonic Postpartum Hemorrhage from Vaginal Delivery

Uterotonic agents were standard drugs used to prevent atonic postpartum hemorrhage, which have different of drugs administration, effectiveness and side effects. This research aimed to compare effectiveness of combined oxytocin-misoprostol, oxytocin and carbetocin in preventing atonic postpartum hemorrhage from vaginal delivery of pregnant women with anemia and to study side effects of drugs. A prospective randomized controlled trial was studied in 324 women with singleton pregnancies who had a vaginal delivery with anemia. Subjects were randomly compared in three groups, 108 patients per group: combined oxytocin-misoprostol group, oxytocin group and carbetocin group. It was found that the beginning of delivery until the end were blood loss ≥ 500 ml in 1, 10 and 2 cases, respectively (p = 0.003). The side effects of drugs were fever in 27, 11 and 14 cases, respectively (p = 0.007), and metallic taste in 26, 2 and 1 cases, respectively (p = <0.001). Combined oxytocin-misoprostol is effective in reduce the blood loss after vaginal delivery in women with anemia, but at the same time, there are still side effects of fever and metallic taste
