Abstract

Total knee arthroplasty (TKA) has become the gold standard to manage the pain and disability associated with knee  posteoarthritis after patients fail nonoperative treatments.  However, significant amount of blood loss may increase the frequency of allogeneic blood transfusion rates.  fortunately, allogeneic blood transfusions have associated  risks including postoperative infection, fluid overload,  increased length of hospital stay, and increased mortality.  This randomized controlled study compared effectiveness  of intra-articular injection of tranexamic acid 250 milligram  versus 500 milligram in reducing blood loss and blood transfusion after TKA of primary knee osteoarthritis  patients at Sukhothai hospital from July, 2018-May, 2019. Of  the 60 patients, the amount of blood volume in the Redivac  drain at 24 hours, 48 hours and blood transfusion were  significantly lower in 500 milligram intra-articular injection  of tranexamic acid group without serious complications,  such as hematoma, deep vein thrombosis, pulmonary  embolism and prosthetic joint infection. In conclusion, 500  milligram intra-articular injection of tranexamic acid is an  effective and safe method.
