บทคัดย่อ

โรคข้ออักเสบรูมาตอยด์และโรคปริทันต์อักเสบเป็นโรคอักเสบเรื้อรังที่พบบ่อย การศึกษาก่อนหน้านี้ แสดงให้เห็นถึงประโยชน์ของการรักษาโรคปริทันต์อักเสบในการลดความรุนแรงของโรคข้ออักเสบรูมาตอยด์ การศึกษาแบบสุ่มและมีกลุ่มควบคุมครั้งนี้มีวัตถุประสงค์เพื่อศึกษาผลของการรักษาโรคปริทันต์แบบไม่ผ่าตัดต่อ ความรุนแรงของโรคข้ออักเสบรูมาตอยด์ ผู้ป่วย 50 คนที่ได้รับการวินิจฉัยโรคข้ออักเสบรูมาตอยด์ที่ยังมีความรุนแรง ระดับปานกลางได้รับการสุ่มให้ได้รับการรักษาโรคปริทันต์ทันที (กลุ่มรักษา) หรือหลังสัปดาห์ที่ 8 (กลุ่มควบคุม) (โดยการขูดหินน้ำลาย, เกลารากฟัน รวมทั้งการแนะนำการดูแลสุขภาพช่องปากการแปรงฟัน, การแปรงซอกฟัน ที่ถูกต้องและคำแนะนำด้านสุขอนามัยช่องปาก) ประเมินความรุนแรงของโรคข้ออักเสบรูมาตอยด์และดัชนีโรคปริทันต์ ก่อนและ 8 สัปดาห์สำหรับผู้เข้าร่วมแต่ละคน ผลลัพธ์หลักพบว่าจำนวนข้อที่บวม, จำนวนข้อที่กดเจ็บ, ระดับของ ความปวด, ค่าการอักเสบ CRP, ESR, ความรุนแรงของโรคข้ออักเสบรูมาตอยด์โดยใช้ DAS28 ESR และ DAS28 CRP ในกลุ่มรักษาลดลง นอกจากนี้ในกลุ่มรักษาดัชนีโรคปริทันต์ดีขึ้นทุกตัวชี้วัดต่างจากในกลุ่มควบคุมที่ไม่ดีขึ้น สรุปได้ว่าการรักษาปริทันต์แบบไม่ผ่าตัดช่วยลดความรุนแรงของโรคข้ออักเสบรูมาตอยด์ได้
