บทคัดย่อ

ภาวะกลืนสิ่งแปลกปลอมโดยเฉพาะก้างปลาพบได้บ่อยในแผนกฉุกเฉินของประเทศไทย ซึ่งการรักษาแบบ ดั้งเดิมมักต้องปรึกษาแพทย์โสตศอนาสิกทำให้อาจล่าช้าและมีภาวะแทรกซ้อนได้ การศึกษากึ่งทดลอง ณ ห้องฉุกเฉิน โรงพยาบาลพิจิตรครั้งนี้จึงมีวัตถุประสงค์เพื่อประเมินประสิทธิภาพของการใช้วิดีโอลาริงโกสโคปโดยแพทย์เวรฉุกเฉิน เปรียบเทียบกับการรักษาแบบดั้งเดิม โดยแบ่งผู้ป่วยที่สงสัยว่ากลืนก้างปลาหรือสิ่งแปลกปลอมเป็นกลุ่มควบคุม (n = 27: ก่อนเริ่มใช้เครื่องมือ) และกลุ่มทดลอง (n=22: หลังเริ่มใช้เครื่องมือ) ผลการศึกษาพบว่ากลุ่มทดลองมีอัตรา การมองเห็นสิ่งแปลกปลอมและคีบออกสำเร็จสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญ (72.7%: 29.6%, p = 0.004 ทั้งสองรายการ) โดยมีค่าจำนวนผู้ป่วยที่ต้องรักษา (NNT) อยู่ที่ 2.3 ราย อย่างไรก็ตาม กลุ่มทดลองมีอัตราการเกิด เลือดออกที่หยุดแล้วสูงกว่า (59.1%: 21.1%, p = 0.009) และใช้ยาปฏิชีวนะมากกว่า (86.4%: 40.7%, p = 0.001) ในขณะที่การส่งต่อแพทย์โสตศอนาสิกลดลง (4.5%: 18.5%, p = 0.204) สรุปได้ว่าแม้มีภาวะแทรกซ้อนเล็กน้อย เพิ่มขึ้น แต่การใช้วิดีโอลาริงโกสโคปโดยแพทย์ฉุกเฉินช่วยเพิ่มอัตราการตรวจพบและคีบสิ่งแปลกปลอมออกได้สำเร็จ อย่างมีนัยสำคัญ ช่วยลดความจำเป็นในการปรึกษาแพทย์เฉพาะทาง จึงควรถือเทคนิคนี้เป็นแนวทางการรักษา เบื้องต้นที่มีประสิทธิภาพสำหรับการจัดการสิ่งแปลกปลอมในทางเดินอาหารส่วนบนในแผนกฉุกเฉิน
