การกลับมารักษาซ้ำในโรงพยาบาลของผู้ป่วยที่ได้รับการดูแลแบบประคับประคอง

การกลับเข้ามารักษาซ้ำในโรงพยาบาลส่งผลกระทบโดยรวมต่อผู้ป่วย ครอบครัว และการใช้บริการทางสาธารณสุขที่ไม่จำเป็น การศึกษาข้อมูลผู้ป่วยย้อนหลังจากเวชระเบียนแล้วเก็บข้อมูลใหม่ไปข้างหน้าครั้งนี้มีวัตถุประสงค์เพื่อประเมินสาเหตุการกลับมารักษาซ้ำในโรงพยาบาลของผู้ป่วยที่ได้รับการดูแลแบบประคับประคอง โดยวิเคราะห์ข้อมูลจากเวชระเบียนและเวชระเบียนอิเล็กทรอนิกส์ผู้ป่วยระยะสุดท้ายที่กลับมารักษาซ้ำในโรงพยาบาลบางระกำระหว่างวันที่ 1 ตุลาคม พ.ศ. 2563–30 กันยายน พ.ศ.2565 จำนวน 157 คน พบว่าเป็นเพศหญิงร้อยละ 52.9, อายุ 61-80 ปีร้อยละ 46.5, เป็นผู้ป่วยมะเร็งร้อยละ 33.8, ระยะเวลานอนโรงพยาบาล 3-7 วันร้อยละ 54.1 และเป็นผู้ป่วยระยะท้าย (PPS 0–30%) ร้อยละ 64.3 สาเหตุการกลับมารักษาซ้ำในโรงพยาบาล ได้แก่ ปัญหาโรคร่วมใหม่ร้อยละ 52.9 (โรคที่พบมาก คือ ไข้/ติดเชื้อในกระแสเลือดร้อยละ 8.9 และติดเชื้อทางเดินปัสสาวะร้อยละ 7.6), ปัญหาด้านการดำเนินโรคร้อยละ 38.9 (พบอาการเหนื่อยร้อยละ 11.5 และอ่อนเพลียร้อยละ 10.2), การวางแผนดูแลผู้ป่วยล่วงหน้าร้อยละ 4.5 และปัญหาด้านครอบครัวร้อยละ 3.8 สรุปได้ว่าสาเหตุการกลับมารักษาซ้ำในโรงพยาบาลของผู้ป่วยที่ได้รับการดูแลแบบประคับประคอง ได้แก่ ปัญหาใหม่ ปัญหาเดิม การวางแผนการดูแลผู้ป่วยล่วงหน้า และปัญหาด้านครอบครัว แต่มีผู้ป่วยอีกจำนวนมากที่มารับการรักษาในโรงพยาบาลโดยไม่จำเป็น ถ้าวางแผนการดูแลผู้ป่วยล่วงหน้าและสื่อสารกับผู้ป่วยและครอบครัวเกี่ยวกับอาการของโรคก็จะสามารถลดการกลับมารักษาซ้ำในโรงพยาบาลได้
