การติดตามอาการไม่พึงประสงค์จากการใช้ยารักษาความดันโลหิตสูงในผู้ป่วย ที่ได้รับยา Renin Angiotensin System (RAS) Blockage (ACEI/ARB) 2 ชนิดร่วมกัน

การใช้ยา ACEIs ร่วมกับ ARBs รักษาความดันโลหิตสูงเป็นหนึ่งในตัวชี้วัดของโครงการโรงพยาบาลส่งเสริมการใช้ยาอย่างสมเหตุผล เนื่องจากไม่สอดคล้องกับแนวทางการรักษาความดันโลหิตสูงในเวชปฏิบัติทั่วไป พ.ศ. 2562 ซึ่งแนะนำว่าไม่ควรใช้ยา ACEIs ร่วมกับ ARBs เพราะเพิ่มความเสี่ยงต่อการเกิดอาการไม่พึงประสงค์จากยาการวิจัยนี้มีวัตถุประสงค์เพื่อเปรียบเทียบการเกิด ภาวะโปแตสเซียมในเลือดสูงระหว่างผู้ป่วยที่ได้รับยา ACEIs ร่วมกับ ARBs รักษาโรคความดันโลหิตสูงกับผู้ป่วยที่ได้รับยา ACEIs หรือ ARBs เพียงกลุ่มเดียว โดยศึกษาข้อมูลย้อนหลังจากฐานข้อมูลเวชระเบียน พบว่าผู้ป่วยเป็นเพศหญิงร้อยละ 49.6 และอายุ มากกว่า 70 ปีร้อยละ 51 กลุ่มผู้ป่วยที่ได้รับยา ACEIs ร่วมกับ ARBs เป็นโรคความดันโลหิตสูงอย่างเดียวร้อยละ 52.2, กลุ่มผู้ป่วยที่ ได้รับยา ACEIs มีโรคไตเรื้อรังร่วมกับความดันโลหิตสูงร้อยละ 35 และกลุ่มผู้ป่วยที่ได้รับยา ARBs มีโรคไตเรื้อรังร่วมกับความดัน โลหิตสูงร้อยละ 52.9 ผู้ป่วยทั้ง 3 กลุ่มประมาณสี่ในห้ามีระดับโปแตสเซียมในเลือดปกติ แต่หนึ่งในสี่ของกลุ่มผู้ป่วยที่ได้รับยา ACEIs ร่วมกับ ARBs มีโปแตสเซียมสูงในระดับไม่รุนแรง แต่ค่ามัธยฐานของระดับโปแตสเซียมในเลือดของผู้ป่วยทั้ง 3 กลุ่มไม่แตก ต่างกัน ดังนั้นการเลือกใช้ยารักษาความดันโลหิตสูงจึงควรพิจารณาจากโรคร่วมของผู้ป่วยและข้อห้ามใช้ของยา ที่สำคัญไม่ควรใช้ ACEIs ร่วมกับ ARBs เนื่องจากเพิ่มความเสี่ยงต่อการเกิดภาวะโปแตสเซียมในเลือดสูงมากกว่าการใช้ยา ACEIs หรือ ARBs เพียง กลุ่มเดียว
