The study of obstructed colon cancer patient with shock: case study

Abstract

 

The obstructed colon cancer is a crisis that must have been urgent fix. The patient with obstructed colon from cancer is usually risk for the lack of fluid and electrolyte or leaky gut syndrome resulting in shock, acute kidney failure and die.

In a case study, the patient, 65 years old, had symptoms with stomach cramps, flatulence and constipation for 4 days before admission to hospital. Doctor diagnosed colonic obstruction R/O CA colon. Therefore, the patient was treated by explor - lap with sigmoid colostomy. Before sigmoid colostomy, the patient was receiving the nursing to fix the shock due to the imbalance of fluid and electrolyte. Further, the increasing efficiency of breathing, reducing of stomach cramps, and anxiety of patients were treated before surgery. After a sigmoid colostomy, the patient may risk for shock. Thus, the nursing care by controlling fluid and electrolyte and drug administration is a solution for fixing the risk. After surgery for 6 days, the patient was receiving the nursing before and after endoscopic examination of the colon for further diagnosis. It was found the cancerous lumps in the anus. Moreover, the progression of cancer was found in the abdominal cavity after x-ray computer determination .In periods of wound infection risk, the correct treatment of pain and wound after surgery resulted that it’s not found the wound infection. The patient and patient relatives were trained for nursing care after colostomy during stay at home. After that, the patient was discharged (12 days of hospital admission). From the follow-up, the complications were not found.

The role of the nurse in the surgical ward is very important in the nursing care. The patient in crisis periods, before shock until after shock, is possible to risk of death. The complications, acute kidney failure, respiratory failure and wound infection of patient were often found. Therefore, the nurse must have the knowledge, skills development and expertise in patient care leading to effective nursing practice corresponding to the problems of each patient and increasing nursing quality.
