บทคัดย่อ
คุณภาพชีวิตในผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายหลังปลูกถ่ายไตเป็นเรื่องสำคัญทางสาธารณสุขที่ควรให้ความสนใจ การศึกษาภาคตัดขวางครั้งนี้มีวัตถุประสงค์เพื่อเปรียบเทียบคุณภาพชีวิตของผู้ป่วยโรคไตวายเรื้อรังระยะสุดท้ายหลังปลูกถ่ายไต กับผู้ป่วยโรคไตเรื้อรังที่ไม่เคยบำบัดทดแทนไต และประชาชนทั่วไป และศึกษาปัจจัยที่มีทำนายคคุณภาพชีวิตของผู้ป่วยโรคไตวายเรื้อรังระยะสุดท้ายหลังปลูกถ่ายไต กลุ่มตัวอย่างถูกคัดเลือกตามเกณฑ์การคัดเข้าของการศึกษา โดยมาจากกลุ่มตัวอย่างที่มารับการรักษาที่โรงพยาบาลศิริราชระหว่าง พฤศจิกายน 2560 – มีนาคม 2561 ข้อมูลถูกเก็บด้วยวิธีการตอบแบบสอบถามด้วยตนเอง และมีการทบทวนเวชระเบียน แบบสอบถามประกอบไปด้วย 4 ส่วน คือ แบบสอบถามวัดคุณลักษณะทั่วไป แบบสอบถามวัดคุณภาพชีวิตในผู้ป่วยโรคไตเรื้อรัง (KDQOL-SFTM) แบบสอบถามวัดการสนับสนุนทางสังคม และแบบสอบถามวัดความพึงพอใจต่อการรักษาของผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายที่ได้รับการปลูกถ่ายไต
ผลการศึกษาพบว่า ผู้ป่วยโรคไตวายเรื้อรังระยะสุดท้ายหลังปลูกถ่ายไตมีคุณภาพชีวิตต่ำที่สุด (p<0.001) เมื่อเทียบกับผู้ป่วยโรคไตเรื้อรังที่ยังไม่ได้รับการบำบัดทดแทนไต และประชาชนทั่วไป จากการวิเคราะห์ถดถอยแบบพหุคูณพบว่า ความพึงพอใจต่อการรักษาที่ได้รับ (B=0.116, p=0.002) ทำให้คุณภาพชีวิตผู้ป่วยดีขึ้นอย่างมีนัยสำคัญทางสถิติ ในขณะที่ผู้ที่มีสถานภาพสมรสโสด/หม้ายหย่า (B= -11.844, p<0.001) หรือผู้ที่มีโรคประจำตัวเป็นโรคความดันโลหิตสูง (B= -18.473, p<0.001) โดยจะมีคุณภาพชีวิตแย่ลง ดังนั้นการเพิ่มประสิทธิผลในการรักษาที่ดีขึ้นเป็นสิ่งสำคัญที่จะเพิ่มคุณภาพชีวิตผู้ป่วยที่ได้รับการปลูกถ่ายไต
