ความสัมพันธ์ของปรากฏการณ์เอลนีโญ-เซาเทิร์นออสซิลเลชันและไดโพลมหาสมุทรอินเดีย กับอุบัติการณ์โรคติดเชื้อระบบทางเดินหายใจ ในพื้นที่จังหวัดเชียงใหม่

โรคติดเชื้อระบบทางเดินหายใจเป็นปัญหาสาธารณสุขที่สำคัญและมีแนวโน้มเพิ่มสูงขึ้นในพื้นที่จังหวัดเชียงใหม่ แต่การศึกษาความสัมพันธ์กับปัจจัยภูมิอากาศระดับโลกอย่างปรากฏการณ์ ENSO และ IOD ในพื้นที่ ยังมีจำกัดและมักเน้นเฉพาะการใช้ดัชนีบ่งชี้สภาพภูมิอากาศในระดับภูมิภาคเท่านั้น การศึกษานี้จึงมีวัตถุประสงค์เพื่อศึกษาแนวโน้ม และความสัมพันธ์ของปรากฏการณ์ ENSO และ IOD กับอุบัติการณ์โรคติดเชื้อระบบทางเดินหายใจ ในพื้นที่จังหวัดเชียงใหม่
การศึกษานี้เป็นการศึกษาแบบ Ecological Time-series Study วิเคราะห์ความสัมพันธ์รายเดือนย้อนหลัง 10 ปี (พ.ศ. 2557–2566) ระหว่างดัชนีบ่งชี้ปรากฏการณ์ ได้แก่ SOI, ONI, MEI.v2 และ DMI จากฐานข้อมูลองค์กร NOAA กับจำนวนผู้ป่วยจากระบบรายงาน 506 ด้วยสถิติการถดถอยทวินามเชิงลบ (Negative Binomial Regression Models) และวิเคราะห์แยกตามช่วงหน่วงเวลา 0–12 เดือน เพื่อประเมินขนาดและทิศทางของความสัมพันธ์เชิงเวลา โดยไม่รวมผลกระทบจากตัวแปรอื่น ๆ
เมื่อวิเคราะห์ความสัมพันธ์ด้วยสถิติการถดถอยทวินามเชิงลบ พบว่าดัชนีบ่งชี้ปรากฏการณ์ ENSO (SOI ONI และ MEI.v2) และ IOD (DMI) มีความสัมพันธ์กับจำนวนโรคติดเชื้อระบบทางเดินหายใจในพื้นที่อย่างมีนัยสำคัญ โดยที่ช่วงเวลาหน่วง 12 เดือน ซึ่งเมื่อค่าดัชนีบ่งชี้ปรากฏการณ์ ENSO (ONI) และ IOD (DMI) ลดลง 1 หน่วย จะส่งผลให้จำนวนผู้ป่วยมีโอกาสเพิ่มขึ้นร้อยละ 32.81 (RR=0.7530, 95%CI=0.6699-0.8463) และ 44.57 (RR=0.6917, 95%CI=0.5245-0.9122) ตามลำดับ
ดังนั้น ดัชนีบ่งชี้ปรากฏการณ์ ENSO และ IOD สามารถประยุกต์ใช้ในการเตรียมมาตรการป้องกันควบคุมโรค และวางแผนจัดสรรทรัพยากรด้านสาธารณสุขได้นานถึง 1 ปี อย่างไรก็ตาม ผลการศึกษานี้เป็นการประมาณความสัมพันธ์ในระดับประชากร ควรมีการวิเคราะห์ร่วมกับปัจจัยด้านชีวภาพของเชื้อ ภูมิคุ้มกัน และพฤติกรรมทางสังคม เพื่อเพิ่มความแม่นยำในการพัฒนาระบบเตือนภัยล่วงหน้า (Early Warning System) ที่สามารถรับมือกับความแปรปรวนของสภาพภูมิอากาศในอนาคต
