Effects of Pharmaceutical Care and Pictorial Medication Labels in Patients with Warfarin at Kamalasai Hospital

Abstract
Objective: To assess knowledge on warfarin use, behavior on warfarin use, opinions/satisfaction towards pictorial medication labels, and International Normalized Ratio (INR) among patients with warfarin before and after receiving pharmaceutical care combined with pictorial medication labels. Methods: This study was a quasi-experimental study. Data were collected from 68 new and existing patients who received warfarin but whose INR was not within the target range at the Warfarin Clinic of Kamalasai Hospital which is a community hospital in Kalasin Province. The study compared outcomes measured before and after pharmaceutical care combined with the use of pictorial medication labels over a 5-month period. Pharmaceutical care included assessment of drug-related problems, dosage adjustment recommendations to physicians, and education with instructions on how to take the medication through pictorial medication labels. The study collected the data on INR and measured knowledge on warfarin use, behavior on warfarin use, opinions/satisfaction towards pictorial medication labels using an assessment form developed within the study. Results: After the intervention, mean knowledge score on warfarin use significantly increased from 5.76 ± 2.68 to 10.65 ± 1.69 out of 15 (P<0.001). Mean warfarin-use behavior significantly increased from 25.26 ± 2.11 to 26.82 ± 0.38 out of 27 (P<0.001). Satisfaction with the pictorial medication labels significantly increased from 34.51 ± 1.34 to 34.85 ± 0.74 out of 35 (P = 0.002). Proportion of patients with INR within the target range significantly increased from 0% to 38.2% (P<0.001). Conclusion: Pharmaceutical care combined with pictorial medication labels effectively promotes knowledge on warfarin use and appropriate warfarin-use behavior. It also significantly increases the rate of INR control within the target range. This approach should be considered for implementation in warfarin clinics to enhance the quality of patient care in the healthcare system.
