Meaning of Drugs from the Perspective of Patients with Diabetes and Hypertension: A Case Study in Phitsanulok Province

Abstract
Objective: To understand the meaning of drug therapy from the viewpoint of patients with diabetes and hypertension. Methods: Qualitative research was conducted through in-depth interviews. Key informants were chosen using a purposive sampling method from those diagnosed with diabetes and hypertension for at least one year and were prescribed a minimum of two drugs for use on a regular basis. The data were collected in Bang Krathum District and Mueang District, Phitsanulok Province, until saturation was reached, involving 31 key informants. The study was conducted from January 2022 to December 2022. Data were analyzed using thematic analysis. Results: The majority of key informants were female (74.19%), with a mean age of 66.16 ± 8.90 years, a mean illness duration of 9.42 ± 4.87 years, and a mean number of regularly used drugs of 5.13 ± 1.45. Most of key informants attributed the meanings of drugs based on their experiences, perceptions, and beliefs. Medication was primarily viewed as a means to control disease symptoms, relieve discomfort, and address bodily abnormalities. Additionally, the meaning of drugs was associated with perceived benefits, drug efficacy, and concerns about potential side effects. All of which significantly influenced their drug use behaviors such as changes of doses, mostly the reduction of the number of pills, changes in the timing of drug administration, or seeking alternative treatments. Conclusion: This study sheds light on the perspectives of individuals living with chronic illnesses regarding the meaning of drugs, which may differ from the perspectives of healthcare professionals. It underscores the importance for medical personnel to comprehend the patient's understanding of drugs and drug-related behaviors among those with chronic conditions. This understanding can be integrated into treatment plans collaboratively developed with the patients.
