บทคัดย่อ

วัตถุประสงค์: เพื่อประเมินและจำแนกระดับความเสี่ยงต่อการเกิดภาวะหัวใจล้มเหลว (heart failure, HF) ใน 5 ปีของผู้ป่วยเบาหวานชนิดที่ 2 (type 2 diabetes mellitus, T2DM) ที่รับการรักษาในคลินิกเบาหวานผู้ป่วยนอก โรงพยาบาลนครพิงค์ โดยใช้คะแนนความเสี่ยง WATCH-DM วิธีการ: การวิจัยเชิงวิเคราะห์แบบย้อนหลังครั้งนี้ดึงข้อมูลจากระบบเวชระเบียนอิเล็กทรอนิกส์ของโรงพยาบาลนครพิงค์ระหว่าง พ.ศ. 2562–2567 โดยมีกลุ่มตัวอย่างเริ่มต้น 13,958 ราย หลังคัดกรองข้อมูลสูญหายของตัวแปร WATCH-DM ทั้ง 10 ตัวแปร และตัดข้อมูลที่ไม่สมเหตุสมผลทางชีววิทยา คงเหลือกลุ่มตัวอย่างที่มีข้อมูลครบถ้วน 2,382 ราย (ร้อยละ 17.1) การวิเคราะห์ทางสถิติระหว่างกลุ่มความเสี่ยงใช้ Kruskal-Wallis test สำหรับตัวแปรต่อเนื่อง และ Chi-squared test สำหรับตัวแปรเชิงหมวดหมู่ ผลการวิจัย: กลุ่มตัวอย่าง 2,382 ราย มีอายุเฉลี่ย 64.3±11.7 ปี คะแนน WATCH-DM เฉลี่ย 7.6±2.9 และ eGFR เฉลี่ย 72.6±27.4 มล./นาที/1.73 ตร.ม. ตัวอย่างถูกจำแนกออกเป็น 5 กลุ่มความเสี่ยง ดังนี้: ต่ำมาก 1,214 ราย (ร้อยละ 51.0) ต่ำ 574 ราย (ร้อยละ 24.1) ปานกลาง 206 ราย (ร้อยละ 8.6) สูง 330 ราย (ร้อยละ 13.9) และสูงมาก 58 ราย (ร้อยละ 2.4) ผู้ป่วยกลุ่มเสี่ยงสูงและสูงมากจำนวนรวม 388 ราย (ร้อยละ 16.3) ต้องการการดูแลเชิงรุกจากเภสัชกร ผู้ป่วยกลุ่มเสี่ยงในระดับต่าง ๆ มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (P < 0.001) ในตัวแปรอายุ ความดันโลหิตซิสโทลิก eGFR ระดับ HDL-C และประวัติกล้ามเนื้อหัวใจตาย สรุป: คะแนนความเสี่ยง WATCH-DM สามารถนำมาใช้จำแนกระดับความเสี่ยงต่อ HF ในผู้ป่วย T2DM ในบริบทคลินิกผู้ป่วยนอกของไทยได้ โดยผู้ป่วย 1 ใน 6 ราย (ร้อยละ 16.3) อยู่ในกลุ่มที่ต้องการการบริบาลทางเภสัชกรรมเชิงรุกจากเภสัชกร ผลการศึกษานี้สนับสนุนบทบาทสำคัญของเภสัชกรคลินิกในการป้องกันภาวะแทรกซ้อนโรคหัวใจและหลอดเลือดในผู้ป่วยเบาหวาน

Miss Khanita Duangchaemkarn received her Pharm.D. degree in Pharmaceutical Care from Naresuan University, Phitsanulok, Thailand and completed her Ph.D. in Biomedical Engineering at Biomedical Engineering Institute, Chiang Mai University. Currently, she is a lecturer at Division of Pharmacy Practice, School of Pharmaceutical Sciences, University of Phayao, Phayao, Thailand.

Her research specialization is in Biomedical Data Engineering and Processing, specifically artificial intelligence and machine learning in clinical settings, predictive model in healthcare, and translational engineering in health and medicine.

She is very active in the international arena with the leadership positions in the IEEE. She is currently an active volunteer for IEEE Women in Engineering (WIE) Thailand Section, and WIE global level, IEEE Thailand Section Committee member, and Digital Media Ambassador for IEEE Engineering in Medicine and Biology Society, the world’s largest technical society of biomedical engineering, since 2017.
