ปัจจัยที่มีความสัมพันธ์กับความร่วมมือในการใช้ยาของผู้สูงอายุโรคเบาหวานชนิดที่ 2 ในอำเภอคีรีมาศ จังหวัดสุโขทัย: การวิจัยภาคตัดขวาง

บทคัดย่อ
วัตถุประสงค์: เพื่อศึกษาระดับความร่วมมือในการใช้ยาและปัจจัยที่มีความสัมพันธ์กับความร่วมมือในการใช้ยาของผู้สูงอายุโรคเบาหวานชนิดที่ 2 ในอำเภอคีรีมาศ จังหวัดสุโขทัย วิธีการ: การวิจัยเชิงวิเคราะห์แบบภาคตัดขวาง ครั้งนี้ทำในผู้ป่วยโรคเบาหวานชนิดที่ 2 อายุ 60 ปีขึ้นไป จำนวน 374 คน ที่เข้ารับบริการผู้ป่วยนอก ณ โรงพยาบาลคีรีมาศ จังหวัดสุโขทัย ผู้ป่วยตอบแบบประเมินความร่วมมือในการใช้ยาสำหรับคนไทย (Medication Adherence Scale in Thais: MAST-8) และแบบสอบถามปัจจัยที่เกี่ยวข้องตามกรอบแนวคิดพหุมิติของ World Health Organization ผลการศึกษา: ผู้ร่วมการวิจัยจำนวน 324 คน (ร้อยละ 86.6) มีความร่วมมือในการใช้ยาในระดับที่เพียงพอ ปัจจัยที่มีความสัมพันธ์กับความร่วมมือในการใช้ยาอย่างมีนัยสำคัญทางสถิติ ได้แก่ ความสัมพันธ์ที่ดีระหว่างบุคลากรทางการแพทย์กับผู้ป่วย (adjusted odds ratio (aOR) = 2.569, 95% CI 1.448-4.559, P = 0.001) การมีโรคร่วมเป็นโรคเกาต์ (aOR = 0.271, 95% CI 0.083-0.882, P = 0.030) การเคยเข้ารับการรักษาในโรงพยาบาลจากภาวะแทรกซ้อนของความดันโลหิตสูง (adjusted OR = 0.235, 95% CI 0.092-0.602, P = 0.003) ความเชื่ออย่างเหมาะสมต่อการใช้ยา (adjusted OR = 3.398, 95% CI 1.226-9.417, P = 0.019) และระดับ HbA1c ≥ 7.0% (adjusted OR = 0.474, 95% CI 0.238-0.942, P = 0.033) สรุป: ความสัมพันธ์ที่ดีระหว่างบุคลากรทางการแพทย์กับผู้ป่วย และความเชื่ออย่างเหมาะสมต่อการใช้ยา มีความสัมพันธ์เชิงบวกกับความร่วมมือในการใช้ยา ขณะที่การมีโรคร่วมบางชนิดและการควบคุมระดับน้ำตาลที่ไม่ดีมีความสัมพันธ์ในทิศทางลบ ดังนั้น บุคลากรทางการแพทย์ควรส่งเสริมสัมพันธภาพกับผู้ป่วย และสนับสนุนความเชื่อที่ถูกต้องเกี่ยวกับการใช้ยา เพื่อเพิ่มความร่วมมือในการใช้ยาและประสิทธิภาพในการควบคุมโรค
