ปัญหาและอุปสรรคในการให้บริการกัญชาทางการแพทย์แก่ผู้ป่วยที่ได้รับการดูแลแบบประคับประคอง: มุมมองของบุคลากรทางการแพทย์ในจังหวัดปัตตานี

บทคัดย่อ

วัตถุประสงค์: เพื่อศึกษากระบวนการทำงาน ปัญหาและอุปสรรคในระบบการให้บริการกัญชาทางการแพทย์แก่ผู้ป่วยที่ได้รับการดูแลแบบประคับประคองในอำเภอเมือง จังหวัดปัตตานี และนำเสนอแนวทางการพัฒนาระบบการให้บริการดังกล่าว วิธีการ: ผู้วิจัยสัมภาษณ์เจาะลึกผู้ปฏิบัติงานที่เกี่ยวข้องกับการให้บริการกัญชาทางการแพทย์และการดูแลผู้ป่วยแบบประคับประคองในอำเภอเมือง จังหวัดปัตตานี 15 ท่าน และสังเกตการให้บริการ จัดการสนทนากลุ่มผู้บริหารและผู้ปฏิบัติงาน 8 ท่านเพื่อจัดทำแนวทางการพัฒนาระบบบริการ การเก็บข้อมูลดำเนินการระหว่าง มีนาคม-กันยายน พ.ศ. 2567 การวิเคราะห์ข้อมูลใช้การวิเคราะห์แก่นสาระ ผลการวิจัย: การให้บริการคลินิกกัญชาทางการแพทย์เป็นระบบบริการแบบสหสาขาวิชาชีพ เชื่อมโยงระหว่างโรงพยาบาลและโรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) ครอบคลุมการคัดกรอง วินิจฉัย สั่งใช้ยา ให้คำแนะนำ และติดตามผล โดยมีการส่งต่อข้อมูลกับชุมชนเพื่อให้ผู้ป่วยได้รับการรักษาอย่างเหมาะสมและต่อเนื่อง ปัญหาในระบบการให้บริการกัญชาทางการแพทย์แบ่งเป็น 7 ด้าน คือ 1) ด้านบุคลากร พบข้อจำกัดด้านความรู้ ประสบการณ์ ความมั่นใจ และจำนวนกำลังคนไม่เพียงพอ 2) ด้านยา ยากัญชาอายุสั้น รูปแบบยาหยดใช้งานยาก และการหยุดสนับสนุนยา 3) ด้านแนวทางการรักษา ยากัญชาไม่ใช่ยาหลักและแนวทางปฏิบัติขัดแย้งกับนโยบายที่ต้องการเพิ่มการเข้าถึงยา 4) ด้านสถานที่และสิ่งแวดล้อม ยังไม่เอื้อต่อการให้บริการ 5) ด้านระบบงาน ขาดความเชื่อมโยงระหว่างผู้ปฏิบัติงานและระหว่างโรงพยาบาลกับ รพ.สต. 6) ด้านการประชาสัมพันธ์ ประชาสัมพันธ์น้อยและขาดสื่อที่เป็นภาษาท้องถิ่น และ 7) ด้านผู้รับบริการ จำนวนลดลง ขาดความรอบรู้เรื่องกัญชา มีพฤติกรรมที่สร้างความไม่สบายใจแก่บุคลากร ปฏิเสธยากัญชา และการใช้กัญชาขัดกับความเชื่อของผู้ป่วย แนวทางการพัฒนาการให้บริการกัญชาทางการแพทย์ได้แก่ การสร้างความรอบรู้และความเชื่อมั่นแก่เจ้าหน้าที่ การปรับปรุงกระบวนการทำงานและเชื่อมโยงการจัดบริการระหว่างหน่วยงาน การพัฒนาสถานที่ให้บริการ การบริหารจัดการระบบยา การเพิ่มการประชาสัมพันธ์ การสร้างความรอบรู้เรื่องกัญชาแก่ประชาชน และการวิจัยเกี่ยวกับการใช้กัญชาทางการแพทย์ สรุป: ปัญหาในการให้บริการกัญชาทางการแพทย์มี 7 ด้าน การแก้ไขปัญหาควรทำโดยการสร้างความรู้และ ความมั่นใจของบุคลากร เตรียมความพร้อมด้านคน ยา และสถานที่ให้บริการ ปรับปรุงกระบวนการทำงาน เพิ่มการประชาสัมพันธ์ และสร้างความรอบรู้และความเชื่อมั่นของประชาชนในการใช้กัญชาทางการแพทย์อย่างเหมาะสมและปลอดภัย
