การพัฒนาแดชบอร์ดสำหรับงานกำกับดูแลการใช้ยาต้านจุลชีพอย่างเหมาะสมในโรงพยาบาล: กรณีศึกษาโรงพยาบาลอ่างทอง

บทคัดย่อ
วัตถุประสงค์: เพื่อพัฒนาแดชบอร์ดสำหรับงานกำกับดูแลการใช้ต้านจุลชีพอย่างเหมาะสมของโรงพยาบาลอ่างทอง วิธีการ: ผู้วิจัยรวบรวมและวิเคราะห์ตัวชี้วัดที่เกี่ยวข้องกับการส่งเสริมการใช้ยาต้านจุลชีพอย่างเหมาะสมจาก 3 แหล่งข้อมูล ได้แก่ 1) antimicrobial stewardship programmes in health-care facilities in low- and middle-income countries: a WHO practical toolkit 2) ตัวชี้วัดงานการจัดการการดื้อยาต้านจุลชีพของกระทรวงสาธารณสุข และ 3) การสัมภาษณ์เชิงลึกเภสัชกรผู้ปฏิบัติงานด้านการกำกับดูแลการใช้ยาต้านจุลชีพในโรงพยาบาลอ่างทองจำนวน 5 ราย การสัมภาษณ์ยังทำเพื่อสำรวจความคิดเห็นและความต้องการต่อการพัฒนาแดชบอร์ด จากนั้นผู้วิจัยสรุปชุดตัวชี้วัดและประเภทข้อมูลที่จำเป็นต่อการรายงานบนแดชบอร์ด ผู้วิจัยสืบค้นข้อมูลจากระบบสารสนเทศโรงพยาบาลและพัฒนาระบบจัดเก็บข้อมูลเพิ่มเติมสำหรับข้อมูลที่ไม่ปรากฏในระบบเดิม ข้อมูลทั้งหมดถูกนำไปจัดเก็บในคลังข้อมูลบน Google Sheet ก่อนดำเนินการพัฒนาแดชบอร์ดด้วยโปรแกรม Looker Studio หลังจากนั้น นำแดชบอร์ดที่พัฒนาขึ้นให้เภสัชกรผู้เชี่ยวชาญด้านโรคติดเชื้อและการใช้ยาต้านจุลชีพ เภสัชกรผู้เชี่ยวชาญด้านสารสนเทศทั่วไป และเภสัชกรผู้เชี่ยวชาญด้านสารสนเทศในโรงพยาบาล จำนวน 3 รายประเมินประสิทธิภาพการทำงาน จากนั้นจึงนำแดชบอร์ดให้เภสัชกรที่รับผิดชอบงานกำกับการใช้ยาต้านจุลชีพของโรงพยาบาลอ่างทองจำนวน 5 ราย ทดลองใช้งานเป็นเวลา 3 เดือน และประเมินความพึงพอใจต่อระบบ ผลการวิจัย: ตัวชี้วัดสำคัญที่ควรแสดงบนแดชบอร์ดประกอบด้วย 10 ด้าน ได้แก่ 1) ภาพรวมการติดตามการใช้ยาต้านจุลชีพอย่างเหมาะสม 2) การประเมินความเหมาะสมในการใช้ยา ครั้งที่ 1 (ภายใน 7 วัน) 3) การประเมินความเหมาะสมในการใช้ยา ครั้งที่ 2 (หลัง 7 วัน) 4) DDD per 1,000 patient-days 5) ปริมาณการใช้ยาต้านจุลชีพรายปี 6) มูลค่าการใช้ยาต้านจุลชีพรายปี 7) อัตราการติดเชื้อ Clostridium difficile 8) การติดเชื้อที่ดื้อยาต้านจุลชีพหลายขนานตามตัวชี้วัดใน service plan AMR ของกระทรวงสาธารณสุข 9) อัตราการกลับเข้ารักษาซ้ำภายใน 30 วันจากโรคติดเชื้อ และ 10) อัตราการตายในโรงพยาบาลที่เกี่ยวข้องกับการติดเชื้อ ในการประเมินความพึงพอใจของผู้ใช้งาน (เภสัชกร) พบว่า มีความพึงพอใจต่อเนื้อหาในแดชบอร์ด 3.93±0.80 ความถูกต้องของข้อมูล 4.30±0.35 รูปแบบการนำเสนอ 4.10±0.74 ความง่ายในการใช้งาน 3.80±0.84 และความสม่ำเสมอในการปรับปรุงข้อมูล 4.00±0.71 โดยความพึงพอใจโดยรวมอยู่ในระดับ 4.00±0.71(จากคะแนนเต็ม 5) สรุป: เภสัชกรผู้ใช้งานแดชบอร์ดที่พัฒนาขึ้นมีความพึงพอใจในระดับสูงในด้านเนื้อหา ความถูกต้องของข้อมูล รูปแบบการนำเสนอ ความง่ายในการใช้งาน และการปรับปรุงข้อมูลอย่างต่อเนื่อง แดชบอร์ดมีศักยภาพในการสนับสนุนการตัดสินใจในงานกำกับดูแลการใช้ยาในงานส่งเสริมการใช้ยาต้านจุลชีพอย่างเหมาะสม
