การขับเคลื่อนนโยบายการแพทย์แผนไทยและการแพทย์ทางเลือกในระดับจังหวัดด้วยแนวคิดการอภิบาลระบบ

บทคัดย่อ
วัตถุประสงค์: เพื่อศึกษาการขับเคลื่อนนโยบายการแพทย์แผนไทยและการแพทย์ทางเลือกของผู้ปฏิบัติงานในกลุ่มงานการแพทย์แผนไทยและการแพทย์ทางเลือก สำนักงานสาธารณสุขจังหวัดด้วยกรอบแนวคิดเรื่องการอภิบาลระบบ วิธีการ: งานวิจัยเชิงคุณภาพครั้งนี้เก็บข้อมูลผ่านการสัมภาษณ์กลุ่มแบบกึ่งโครงสร้างในผู้ปฏิบัติงานในกลุ่มงานการแพทย์แผนไทยและการแพทย์ทางเลือกจาก 12 จังหวัด ประกอบด้วย แพทย์แผนไทย เภสัชกร และนักวิชาการสาธารณสุข จำนวน 40 คน ระหว่างเดือนตุลาคม 2566 ถึง กุมภาพันธ์ 2567 การศึกษาพัฒนาแนวคำถามการขับเคลื่อนงานตามนโยบายที่ได้รับมอบหมาย โดยใช้กรอบแนวคิดการอภิบาลระบบ ได้แก่ 1) การกำหนดนโยบายและแผนยุทธศาสตร์ 2) การสร้างปัญญา 3) การวางกลไกหรือเครื่องมือในการดำเนินนโยบาย 4) ความร่วมมือและพันธมิตร และ 5) การประกันภาระความรับผิดชอบ โดยขอให้ผู้ปฏิบัติงานแต่ละจังหวัดอธิบายกระบวนการที่กลุ่มงานใช้สนับสนุนนโยบายการบริการด้านการแพทย์แผนไทยและการแพทย์ทางเลือกและนโยบายเมืองสมุนไพร ผลการวิจัย: การดำเนินงานขับเคลื่อนนโยบายการแพทย์แผนไทยและการแพทย์ทางเลือกระดับจังหวัดมีความครอบคลุมตามกรอบแนวคิดการอภิบาลระบบครบทั้ง 5 ด้าน โดยพบความโดดเด่นด้านความร่วมมือและพันธมิตรทั้งจากภาคเอกชนหรือภาคประชาชน เช่น กลุ่มหมอพื้นบ้านที่เป็นแหล่งข้อมูลสำคัญในจังหวัด กลุ่มผู้ประกอบการด้านการท่องเที่ยวในการสนับสนุนนโยบายเศรษฐกิจสุขภาพ และองค์การบริหารส่วนจังหวัดเพื่อให้การจัดบริการการแพทย์แผนไทยต่อเนื่องในสถานการณ์การกระจายอำนาจสู่ท้องถิ่น นอกจากนี้ ยังพบการวางกลไกหรือเครื่องมือในการทำงานโดยการกำหนดแนวทางการทำงานให้สอดคล้องกับการปฏิบัติงานในพื้นที่และสร้างองค์ความรู้เพื่อสนับสนุนการทำงาน ในส่วนของจังหวัดเมืองสมุนไพรมีการกำหนดนโยบายและบทบาทของคณะกรรมการระดับจังหวัดที่ชัดเจนมากกว่าจังหวัดที่ไม่ได้เป็นเมืองสมุนไพร ซึ่งเป็นไปตามนโยบายเมืองสมุนไพรจากส่วนกลาง สรุป: ภายหลังจากรับนโยบายจากส่วนกลาง การดำเนินงานของกลุ่มงานการแพทย์แผนไทยและการแพทย์ทางเลือกในระดับจังหวัดมีการวางกลไกการทำงานตามบริบทของพื้นที่ ผลการทำงานที่ดีมีความสัมพันธ์กับความร่วมมือทั้งในและนอกภาครัฐและการสร้างความรู้เพื่อใช้ข้อมูลในการตัดสินใจ
