มุมมองของเภสัชกรต่อบทบาทที่ยังมีช่องว่างในการบริบาลเภสัชกรรมด้านการตรวจเยี่ยมผู้ป่วยบนหอผู้ป่วยและการติดตามวัดระดับยาในเลือด : การศึกษาเชิงคุณภาพในโรงพยาบาลระดับตติยภูมิแห่งหนึ่ง

บทคัดย่อ
วัตถุประสงค์: เพื่อศึกษามุมมองของเภสัชกรที่ปฏิบัติงานในโรงพยาบาลระดับตติยภูมิแห่งหนึ่งในจังหวัดขอนแก่นต่อบทบาทในการบริบาลเภสัชกรรม วิธีการ: การศึกษานี้เป็นการวิจัยเชิงคุณภาพที่ใช้การสัมภาษณ์เชิงลึกในผู้ให้ข้อมูลสำคัญจำนวน 18 คน ประกอบด้วยเภสัชกรที่ปฏิบัติงานประจำบนหอผู้ป่วยอายุรกรรมทั่วไปและหอผู้ป่วยมะเร็ง และเภสัชกรที่ไม่ได้ปฏิบัติงานประจำบนหอผู้ป่วย โดยมาจากหลายแผนกในกลุ่มงานเภสัชกรรม หลายบทบาทหน้าที่ และมีระดับประสบการณ์ที่แตกต่างกัน การศึกษาเก็บข้อมูลระหว่างวันที่ 1 ตุลาคม ถึง 15 พฤศจิกายน 2567 การวิเคราะห์ข้อมูลใช้การวิเคราะห์เนื้อหา ผลการวิจัย: ผู้ให้ข้อมูลสำคัญมีมุมมองเชิงบวกต่อบทบาทของตนในการตรวจเยี่ยมผู้ป่วย (ward rounds) และตระหนักถึงความสำคัญของกิจกรรม โดยเฉพาะผู้ที่ปฏิบัติงานประจำบนหอผู้ป่วยเห็นถึงประโยชน์ของการมีส่วนร่วมในการวางแผนการใช้ยากับทีมบุคลากรทางการแพทย์ อย่างไรก็ตามการมีส่วนร่วมยังคงจำกัด เนื่องจากภาระงานที่สูง อัตรากำลังไม่เพียงพอ และการขาดระบบหรือกลไกที่ชัดเจนในการตรวจเยี่ยมผู้ป่วยร่วมกับทีมสหสาขาวิชาชีพ สำหรับการติดตามวัดระดับยาในเลือดพบว่า เภสัชกรหลายคนยังไม่มั่นใจในความรู้เฉพาะทาง บุคลากรที่รับผิดชอบด้านนี้โดยตรงมีน้อย และระบบสนับสนุนยังไม่สมบูรณ์ทั้งในด้านข้อมูลและเทคโนโลยี อย่างไรก็ดี ผู้ที่มีประสบการณ์หรือผ่านการอบรมสามารถปฏิบัติงานได้อย่างมีประสิทธิภาพและได้รับการยอมรับจากทีมบุคลากรทางการแพทย์ สรุป: บทบาทของเภสัชกรในการบริบาลเภสัชกรรม โดยเฉพาะการตรวจเยี่ยมผู้ป่วยและการติดตามวัดระดับยาในเลือด ยังมีข้อจำกัดและไม่สอดคล้องกับความคาดหวังของบุคลากรทางการแพทย์ เภสัชกรเสนอแนวทางพัฒนา เช่น การเพิ่มบุคลากร ระบบตรวจเยี่ยมที่ชัดเจน การฝึกอบรมเฉพาะทาง การจัดตั้งหน่วยติดตามวัดระดับยาในเลือด และการใช้เทคโนโลยี เพื่อลดช่องว่างที่เกิดขึ้นและยกระดับคุณภาพการในดูแลผู้ป่วยโดยรวม
