บทคัดย่อ
วัตถุประสงค์: เพื่อเสนอแนวทางการพัฒนาระบบบริการคลินิกชะลอไตเสื่อมในโรงพยาบาลน้ำพอง จังหวัดขอนแก่น โดยประยุกต์ใช้วิธีซอฟท์ซิสเต็ม (soft system methodology: SSM) วิธีการ: ผู้วิจัยทำความเข้าใจสถานการณ์ปัจจุบันของบริการโดยสังเกตระบบการให้บริการ รวบรวมข้อมูลจากเอกสาร สำรวจข้อมูลปัญหาที่เกี่ยวกับยา และสัมภาษณ์เชิงลึกหรือสนทนากลุ่มในผู้ให้ข้อมูลสำคัญ 3 กลุ่ม ได้แก่ ผู้บริหารและหัวหน้างานที่เกี่ยวข้อง 6 คน ทีมสหวิชาชีพ 7 คน และผู้ป่วยโรคไตเรื้อรังที่มีปัญหาเกี่ยวกับยา 19 คน การศึกษาวิเคราะห์ข้อมูลเชิงเนื้อหาโดยใช้แนวคิดเกี่ยวกับองค์ประกอบของคุณภาพบริการทางสุขภาพของ Donabedian ได้แก่ ด้านโครงสร้าง ด้านกระบวนการ และด้านผลลัพธ์ รวมถึงใช้ความคาดหวังของผู้ให้ข้อมูลสำคัญเป็นกรอบในการวิเคราะห์ข้อมูล การวิเคราะห์ปัญหาที่เกี่ยวกับยาใช้กรอบที่กำหนดโดย Pharmaceutical Care Network Europe เวอร์ชั่น 7.0 ผู้วิจัยนำข้อมูลที่ได้มาพิจารณาประกอบการร่างภาพสถานการณ์ปัจจุบันและคำอธิบายเป้าหมายเชิงระบบ ออกแบบระบบกิจกรรม เปรียบเทียบระบบกิจกรรมกับกิจกรรมที่ทำในปัจจุบัน พิจารณาความเป็นไปได้ในการพัฒนาบริการภายใต้บริบทปัจจุบัน และเสนอแนวทางการพัฒนาบริการ ผลการวิจัย: แนวทางการพัฒนาระบบบริการคลินิกชะลอไตเสื่อมในโรงพยาบาลน้ำพอง ประกอบด้วยแนวทางระยะสั้น 5 แนวทางและแนวทางระยะยาว 4 แนวทาง สรุป: SSM เป็นวิธีการเชิงระบบที่ช่วยเสนอแนวทางการพัฒนาระบบบริการคลินิกชะลอไตเสื่อมในโรงพยาบาลน้ำพองได้อย่างครอบคลุมผ่านการมีส่วนร่วมของผู้เกี่ยวข้อง จึงสามารถนำแนวทางที่ได้ไปดำเนินการในลักษณะงานวิจัยเชิงปฏิบัติการอย่างมีส่วนร่วมต่อไป
